I. The Science of Handoffs

End-of-shift handoffs transfer responsibility between day and night teams and occur at least twice daily for inpatients. Cross-coverage handoffs prepare covering physicians to care for unfamiliar patients during weekends, holidays, or call periods. Service transfer handoffs move patients between care teams such as from the emergency department to the inpatient ward or from the intensive care unit to the floor. Discharge handoffs transition care responsibility from the hospital team to outpatient providers, requiring particularly thorough communication to bridge care settings.

Barriers to effective handoffs create opportunities for error that must be actively addressed through environmental and process controls. II. The I-PASS Framework

Illness severity classification opens the I-PASS handoff by immediately orienting the receiver to the patient's acuity level and monitoring needs. Stable patients are following expected clinical courses and require routine care without special vigilance. Watcher patients have potential for deterioration or are in clinical situations requiring increased attention without current instability. Unstable patients have active clinical concerns requiring close monitoring and potentially rapid intervention. This upfront classification helps receiving providers prioritize among multiple patients and allocate attention appropriately.

Patient summary provides the clinical context necessary for understanding the current situation and making appropriate decisions. The one-liner establishes patient identity through brief description including age, relevant history, and reason for admission. Key events summarize what has happened during hospitalization that the receiver needs to know. Current status describes where the patient is now in their clinical trajectory, including recent changes and current stability. This summary should be concise yet complete enough to enable informed decision-making.

Action list specifies the tasks that need to be completed, preventing important items from being overlooked during cross-coverage. Pending to-do items should be explicitly listed with clear descriptions of what needs to be done. Timing expectations clarify when tasks should be completed, distinguishing urgent from routine items. Ownership assignment ensures clarity about whether the outgoing or incoming provider is responsible for each item. Without explicit action lists, tasks are frequently overlooked when they are assumed to be someone else's responsibility.

Situation awareness and contingency planning and synthesis complete the I-PASS framework by preparing for potential changes and confirming understanding. Situation awareness involves explicitly identifying what could go wrong and what signs would indicate deterioration. If-then contingency planning specifies what actions to take if anticipated problems develop, reducing decision-making burden during acute situations. Escalation triggers identify specific thresholds for calling senior residents, attendings, or rapid response teams. Synthesis has the receiver summarize key information back, confirming accurate reception and providing opportunity for correction.

III. Communicating with Patients

IV. Family Communication

Setting a clear agenda focuses discussion on the most important issues and manages time effectively. Arranging a private, comfortable environment supports difficult conversations and demonstrates respect for the gravity of the discussion.

Conducting meetings effectively follows a structured approach that addresses information, emotion, and planning needs. Opening with introductions and purpose statement orients participants and establishes the meeting's focus. V. Difficult Conversations

Breaking bad news requires a structured approach that delivers information compassionately while supporting patient emotional needs. Perception assessment asks what the patient already knows or suspects, calibrating the conversation starting point. Invitation requests permission to share information and assesses how much detail the patient wants. Knowledge delivery provides information in clear, understandable language with appropriate pacing. Emotion response acknowledges and validates feelings with empathetic statements before moving forward. Strategy and summary discuss next steps and confirm patient understanding of key points.

Responding to emotions represents one of the most challenging and important communication skills in medicine. Naming emotions explicitly acknowledges what the patient or family appears to be feeling. Understanding statements validate that the emotional response makes sense given the circumstances. Respect statements acknowledge patient strengths, efforts, or coping through difficult situations. Support statements commit to ongoing presence and assistance regardless of clinical outcomes. Exploring emotions invites patients to share more about their experience, deepening connection and understanding.

Goals of care conversations address treatment preferences for patients with serious illness or complex medical conditions. Prognostic information should be shared honestly and compassionately, avoiding both false hope and premature despair. Eliciting values helps understand what matters most to the patient beyond simply living longer. Presenting options includes all reasonable approaches including comfort-focused care when appropriate. Offering recommendations when asked provides guidance while respecting that final decisions belong to patients. Medical error disclosure requires honest communication that prioritizes patient welfare while managing institutional considerations. Constructive feedback should be delivered privately, promptly, and focused on behavior rather than character. Framing feedback around impact rather than intent helps receivers understand consequences without feeling attacked. Suggesting specific alternatives gives actionable guidance for improvement rather than simply identifying problems.

VII. Consultation Communication

Requesting consultations effectively provides consultants with information needed to respond helpfully and promptly. Stating a specific clinical question focuses the consultation and enables targeted evaluation. Specifying urgency helps consultants prioritize among competing demands and respond within clinically appropriate timeframes. Providing a brief, relevant clinical summary gives context without overwhelming with unnecessary detail. Including contact information ensures that consultants can reach the requesting provider with questions or recommendations.

The five C's of consultation provide a framework for complete, professional consultation communication. Contact means reaching the consultant directly, physician-to-physician, rather than simply placing electronic orders. Communicate involves stating the clinical question clearly and specifically. Core information provides essential data the consultant needs to address the question. Collaborate means working together with the consultant rather than simply receiving recommendations passively. Close the loop ensures that consultation results are received, understood, and incorporated into patient care.

Being an effective consultant involves responding to consultation requests professionally and helpfully. Responding promptly demonstrates respect for patient needs and collegial relationships. Directly answering the specific question asked addresses the consulting team's actual need. Providing clear, actionable recommendations that can be implemented improves patient care. Following up as clinically indicated ensures that recommendations achieve intended effects and that additional guidance is available if needed.

Managing disagreements with consultants requires professional approaches that prioritize patient welfare. Asking consultants to explain their reasoning often resolves apparent disagreements through improved understanding. Sharing your own perspective and concerns creates dialogue rather than unilateral decision-making. Documenting your clinical reasoning when you choose not to follow recommendations creates a clear record of decision-making. Escalating to attending physicians when patient safety concerns remain unresolved ensures appropriate oversight.

VIII. Written Communication

Clinical notes serve multiple purposes that make quality documentation an essential professional skill. Clinical communication ensures that other providers understand the patient's status, assessment, and plan. Legal documentation creates the record that may be reviewed in malpractice proceedings or regulatory investigations. Quality measurement increasingly relies on documentation to assess care processes and outcomes. Billing justification requires documentation that supports the level of service billed.

Order and instruction clarity prevents errors that arise from ambiguous or incomplete written communication. Specificity eliminates ambiguity by stating exactly what should be done, when, and how. Completeness includes all necessary parameters such as dose, route, frequency, and duration for medications. Safety verification through double-checking before signing reduces errors from transcription mistakes or cognitive slips. Documentation in the medical record creates a permanent record of what was ordered and why.

Discharge instructions provide patients with the information needed to continue recovery safely after leaving the hospital. Diagnosis explanation helps patients understand what was found during their hospitalization. Medication lists with changes highlighted identify what patients should be taking and how their regimen has changed. Follow-up instructions specify when and with whom patients should follow up, with specific appointments when possible. Warning signs describe symptoms that should prompt return to care, helping patients recognize concerning developments.

Communication to other providers ensures continuity across care settings and providers. Discharge summaries provide comprehensive records of hospital course, diagnoses, treatments, and follow-up needs for outpatient providers. Referral letters explain the reason for referral and specific questions for the consulting provider. Transfer notes provide complete handoffs for patients moving between facilities or units. IX. Technology in Communication

X. Documentation of Communication
