Seminar Outline

Section 1: Men's Preventive Health

Men experience significant health disparities compared to women, with an average life expectancy approximately five years shorter across most populations. The leading causes of death in men are heart disease, cancer, and unintentional injury, all of which are significantly influenced by modifiable risk factors. Men are substantially less likely to seek preventive healthcare, with studies consistently showing lower rates of routine check-ups, cancer screening, and vaccination compared to women. Mental health disparities are particularly concerning, as men have a higher completed suicide rate despite lower rates of diagnosed depression, reflecting both underdiagnosis due to atypical symptom presentation and a reluctance to seek help rooted in traditional masculinity norms. Addressing these disparities requires a proactive approach from the primary care physician, including opportunistic screening at every encounter and creating a clinical environment that encourages men to engage with preventive services.

Screening recommendations for men encompass cardiovascular, metabolic, and cancer screening modalities. Blood pressure should be measured at every clinical encounter, as hypertension is a major modifiable cardiovascular risk factor with a higher prevalence in men. Lipid screening should begin at age forty with earlier initiation for men with cardiovascular risk factors, and diabetes screening is recommended starting at age thirty-five for overweight or obese men. Colorectal cancer screening should begin at age forty-five and continue through age seventy-five using colonoscopy, stool-based testing, or other approved modalities. Low-dose computed tomography for lung cancer screening is indicated for men aged fifty to eighty with a twenty pack-year or greater smoking history who currently smoke or have quit within the past fifteen years. Abdominal aortic aneurysm screening with a one-time ultrasound is recommended for men aged sixty-five to seventy-five who have ever smoked, carrying a USPSTF B recommendation.

Immunization recommendations for adult men follow the Advisory Committee on Immunization Practices schedule and should be reviewed at every visit. Annual influenza vaccination is recommended for all adults. Tetanus-diphtheria-acellular pertussis vaccine should be administered once if not previously received, with tetanus-diphtheria boosters every ten years thereafter. The recombinant zoster vaccine Shingrix is recommended as a two-dose series for adults aged fifty and older, regardless of prior varicella history. Pneumococcal vaccination is recommended for adults aged sixty-five and older, with earlier administration for those with immunocompromising conditions or chronic diseases. COVID-19 vaccination should follow current guidelines. HPV vaccination is recommended through age twenty-six, with shared clinical decision-making for men aged twenty-seven to forty-five who may still benefit, particularly men who have sex with men.

Lifestyle counseling at every visit addresses the major behavioral contributors to men's morbidity and mortality. Tobacco use should be assessed and assistance offered at every encounter using the five A's framework, as smoking is the single most preventable cause of death. Alcohol use should be screened with validated instruments such as the AUDIT-C, with brief intervention provided for hazardous drinking. Diet and exercise counseling focuses on cardiovascular risk reduction, with a goal of at least one hundred fifty minutes of moderate-intensity aerobic activity per week. Safety counseling should address seatbelt use, firearm safety including secure storage, and helmet use during appropriate activities. Mental health screening for depression should be performed routinely, and clinicians should maintain awareness of suicide risk factors, particularly in older men, veterans, and those with substance use disorders.

Section 2: Erectile Dysfunction

Erectile dysfunction affects approximately ten to twenty percent of men overall, with prevalence increasing significantly with age, affecting up to fifty percent of men in their fifties and up to seventy percent in their seventies. The most common underlying cause is vascular insufficiency from atherosclerosis, making erectile dysfunction an important early marker of systemic cardiovascular disease that may precede clinical coronary events by two to five years. Neurological causes include diabetic neuropathy, multiple sclerosis, spinal cord injury, and post-surgical nerve damage following radical prostatectomy. Hormonal causes, particularly testosterone deficiency, contribute in a subset of patients and are suggested by concurrent low libido. Numerous medications can cause or exacerbate erectile dysfunction, including antihypertensives such as thiazide diuretics and beta-blockers, selective serotonin reuptake inhibitors, and antiandrogen agents. Psychological factors including performance anxiety, depression, relationship conflict, and stress may be the primary cause or a significant contributing factor, particularly in younger men with sudden onset of symptoms and preserved morning erections.

Evaluation of erectile dysfunction begins with a comprehensive history that characterizes the onset, frequency, and circumstances of the dysfunction. Gradual onset with progressive worsening suggests an organic vascular or neurological etiology, while sudden onset in the context of a new relationship or stressful life event points toward a psychogenic cause. The preservation of morning erections and erections during masturbation suggests intact vascular and neurological function and supports a psychogenic contribution. A thorough medication review should identify potential pharmacologic contributors, and comorbid conditions including diabetes, hypertension, coronary artery disease, and depression should be assessed and optimized. Physical examination should include assessment of blood pressure, body habitus, secondary sex characteristics, genital examination for penile plaques or testicular abnormalities, and assessment of peripheral pulses. Laboratory evaluation includes fasting glucose and lipid panel to screen for cardiovascular risk factors, and morning total testosterone should be measured when low libido accompanies erectile dysfunction.

Phosphodiesterase type 5 inhibitors are the first-line pharmacotherapy for erectile dysfunction and work by enhancing nitric oxide-mediated vasodilation in the corpus cavernosum. Sildenafil, the prototype agent, is taken thirty to sixty minutes before sexual activity and provides a duration of action of four to five hours, with efficacy dependent on sexual stimulation. Tadalafil has a significantly longer half-life, providing a duration of action of twenty-four to thirty-six hours, and is also available in a daily low-dose formulation that allows spontaneous sexual activity without timing constraints. Vardenafil has a pharmacokinetic profile similar to sildenafil, while avanafil has the fastest onset of action at fifteen to thirty minutes. The absolute contraindication to PDE5 inhibitor use is concurrent nitrate therapy, as the combination can cause severe, potentially fatal hypotension. Alpha-adrenergic blockers used for benign prostatic hyperplasia represent a relative contraindication, and concurrent use requires dose adjustment and temporal separation to minimize the risk of orthostatic hypotension.

For patients who do not respond to or cannot tolerate PDE5 inhibitors, several alternative treatments are available. Testosterone replacement therapy is appropriate only when documented testosterone deficiency is contributing to erectile dysfunction and should not be used empirically. Vacuum erection devices are non-pharmacological options that create negative pressure to draw blood into the corpora cavernosa, with a constriction band placed at the base of the penis to maintain erection, and may be particularly appropriate for men who prefer to avoid medications or have contraindications. Intracavernosal injection of alprostadil or combination agents produces erection within five to twenty minutes and is effective in up to ninety percent of patients, though it requires instruction in self-injection technique. Penile prosthesis implantation is reserved for refractory cases and involves surgical placement of inflatable or malleable devices. Lifestyle modifications including weight loss, regular exercise, smoking cessation, and optimization of underlying cardiovascular risk factors should be recommended to all patients, as they address the root cause and may improve erectile function independently or enhance the response to pharmacotherapy.

Section 3: Benign Prostatic Hyperplasia

Benign prostatic hyperplasia is one of the most common conditions affecting aging men, with histologic evidence of hyperplasia present in approximately fifty percent of men at age fifty and eighty percent by age eighty. The condition results from androgen-dependent proliferation of both stromal and epithelial elements of the transition zone of the prostate, leading to progressive enlargement that can compress the prostatic urethra and obstruct urinary outflow. It is important to understand that prostate size does not directly correlate with symptom severity, as even modestly enlarged glands can cause significant obstruction depending on their configuration relative to the urethra. BPH is not a premalignant condition, but it frequently coexists with prostate cancer, and the evaluation of one condition should include consideration of the other. The clinical significance of BPH lies in its impact on quality of life through bothersome urinary symptoms and its potential to cause complications including urinary retention, recurrent urinary tract infections, bladder stones, and, rarely, renal insufficiency.

Lower urinary tract symptoms associated with BPH are categorized into storage, voiding, and post-void groups. Storage symptoms, also called irritative symptoms, include urinary frequency, urgency, and nocturia, and result from detrusor overactivity secondary to chronic bladder outlet obstruction. Voiding symptoms, also called obstructive symptoms, include weak urinary stream, hesitancy, intermittency, straining to void, and prolonged voiding time, reflecting direct urethral compression by the enlarged prostate. Post-void symptoms include the sensation of incomplete bladder emptying and post-void dribbling. The International Prostate Symptom Score, also known as the AUA Symptom Index, is a validated seven-question instrument that quantifies symptom severity on a scale of zero to thirty-five, with scores of zero to seven indicating mild symptoms, eight to nineteen indicating moderate symptoms, and twenty to thirty-five indicating severe symptoms. An additional bother score on a scale of zero to six captures the subjective impact of symptoms on quality of life and is critical for treatment decisions.

Evaluation of a man presenting with lower urinary tract symptoms includes a focused history, physical examination, and selective laboratory testing. The history should characterize the duration and progression of symptoms, prior treatments, and medication use, particularly anticholinergics and sympathomimetics that may exacerbate symptoms. Digital rectal examination assesses prostate size, consistency, and the presence of nodules that may suggest malignancy, though it systematically underestimates prostate volume. Urinalysis rules out urinary tract infection and hematuria as alternative or coexisting causes of symptoms. Prostate-specific antigen testing should be discussed in the context of shared decision-making, as it provides information about prostate size and may identify men at risk for progression, but also has implications for cancer screening. Serum creatinine is measured when there is concern for renal impairment from chronic obstruction. Post-void residual measurement by bladder ultrasound is indicated when urinary retention is suspected, with volumes above two hundred milliliters suggesting significant retention warranting further evaluation.

The AUA Symptom Score provides an objective framework for guiding treatment decisions and monitoring response to therapy. Men with mild symptoms scoring zero to seven are generally managed with watchful waiting, which involves annual reassessment of symptoms and simple lifestyle modifications including limiting fluid intake before bedtime, reducing caffeine and alcohol consumption, and double voiding to improve bladder emptying. It is important to reassure patients with mild symptoms that BPH progresses slowly in most cases and that many men do not experience significant symptom worsening over time. Moderate symptoms scoring eight to nineteen typically warrant medical therapy, though patient preference and bother level play important roles in the decision. Severe symptoms scoring twenty to thirty-five usually require pharmacologic treatment and may prompt consideration of surgical intervention, particularly when complicated by recurrent urinary retention, refractory hematuria, bladder stones, or renal insufficiency. Serial symptom scoring at follow-up visits provides an objective measure of treatment efficacy and helps guide decisions about treatment escalation or surgical referral.

Section 4: BPH Treatment

Watchful waiting is the appropriate initial management strategy for men with mild BPH symptoms who report minimal bother from their urinary complaints. This approach involves annual reassessment of symptoms using the IPSS questionnaire and monitoring for the development of complications. Lifestyle modifications that may improve mild symptoms include limiting fluid intake in the evening hours, particularly two to three hours before bedtime, avoiding caffeine and alcohol which have diuretic and bladder-irritant properties, practicing double voiding to maximize bladder emptying, and reviewing medications that may worsen symptoms such as over-the-counter decongestants containing pseudoephedrine. Patients should be counseled that the natural history of BPH is variable, with approximately one-third of men experiencing symptom improvement over time, one-third remaining stable, and one-third progressing. This knowledge empowers patients to make informed decisions about when to initiate pharmacotherapy and helps avoid unnecessary treatment in men with acceptable symptoms.

Alpha-adrenergic blockers are the most commonly prescribed first-line pharmacotherapy for symptomatic BPH and provide rapid symptom relief by relaxing smooth muscle in the prostate and bladder neck. Tamsulosin is the most widely used agent due to its uroselectivity for alpha-1A receptors in the prostate, which minimizes systemic vasodilatory effects. Alfuzosin is an extended-release formulation with a favorable hemodynamic profile, while silodosin is the most uroselective agent available but carries a higher rate of retrograde ejaculation. Non-selective agents including doxazosin and terazosin also lower blood pressure and may be advantageous in men with concurrent hypertension, though they carry greater risk of orthostatic hypotension, particularly with the first dose. The onset of action for alpha-blockers is within days to weeks, providing relatively rapid symptomatic improvement. Common side effects include dizziness, orthostatic hypotension, nasal congestion, and retrograde ejaculation, and patients should be counseled about the risk of intraoperative floppy iris syndrome if cataract surgery is anticipated.

Five-alpha reductase inhibitors represent the second major class of BPH pharmacotherapy and work by blocking the conversion of testosterone to dihydrotestosterone, the primary androgen driving prostatic growth. Finasteride at five milligrams daily and dutasteride at 0.5 milligrams daily are the available agents, with dutasteride inhibiting both type 1 and type 2 isoforms of the five-alpha reductase enzyme while finasteride inhibits only type 2. These medications are most effective in men with larger prostates, generally those exceeding thirty to forty grams, and provide benefits including reduction in prostate volume by approximately twenty to thirty percent, decreased risk of acute urinary retention, and reduced need for surgical intervention. The major limitation is the slow onset of action, requiring six to twelve months for full clinical effect. Side effects include decreased libido, erectile dysfunction, and reduced ejaculate volume, occurring in approximately five to eight percent of patients. Importantly, five-alpha reductase inhibitors reduce serum PSA levels by approximately fifty percent, which must be accounted for when interpreting PSA results for prostate cancer screening.

Combination therapy and alternative approaches expand the treatment armamentarium for BPH. The combination of an alpha-blocker with a five-alpha reductase inhibitor has been shown in large clinical trials, including the CombAT and MTOPS studies, to be superior to either agent alone for symptom improvement and reduction of clinical progression in men with larger prostates. Tadalafil at five milligrams daily is FDA-approved for both BPH and erectile dysfunction, making it an attractive option for men with both conditions. Anticholinergic agents such as tolterodine may be cautiously added when storage symptoms predominate and are inadequately controlled by alpha-blockade alone, though urinary retention must be monitored. The beta-3 agonist mirabegron offers an alternative for overactive bladder symptoms with fewer anticholinergic side effects. Surgical intervention, with transurethral resection of the prostate being the traditional gold standard, is indicated for refractory symptoms, recurrent urinary retention despite catheterization, recurrent urinary tract infections, bladder stones, or renal insufficiency attributable to obstruction. Newer minimally invasive procedures including laser ablation, prostatic urethral lift, and water vapor thermal therapy have expanded surgical options with potentially shorter recovery times.

Section 5: Prostate Cancer Screening

Prostate cancer screening with prostate-specific antigen testing remains one of the most debated topics in preventive medicine, with disagreement among major guideline organizations reflecting the complex balance between potential mortality reduction and significant screening-related harms. The potential benefit of PSA screening is a modest reduction in prostate cancer-specific mortality, estimated at one to two deaths prevented per one thousand men screened over ten to fifteen years. However, the harms are substantial: PSA screening has a high false-positive rate leading to unnecessary prostate biopsies with associated pain, infection, and anxiety; it results in significant overdiagnosis of indolent cancers that would never have caused clinical harm during the patient's lifetime, estimated at approximately twenty to fifty percent of screen-detected cancers; and overtreatment of these indolent cancers with surgery or radiation causes morbidity including erectile dysfunction, urinary incontinence, and bowel dysfunction. The USPSTF provides a C recommendation for men aged fifty-five to sixty-nine, indicating that the decision to screen should be individualized through shared decision-making, and a D recommendation against screening in men aged seventy and older.

Shared decision-making is the mandated approach to prostate cancer screening and requires the clinician to present balanced information about the potential benefits and harms of PSA testing. The discussion should explain that PSA testing may detect prostate cancer earlier and reduce the chance of dying from the disease, but that the absolute mortality benefit is small. The clinician must convey that a positive PSA result will lead to further evaluation including prostate biopsy, which carries its own risks, and that some detected cancers may never have caused harm. The patient's values and preferences are central to the decision: some men place high value on the reassurance of a negative test and the possibility of early detection, while others prioritize avoiding the potential harms of false positives and overtreatment. The discussion and the patient's informed decision should be documented in the medical record. This is not a brief, formulaic conversation but rather a thoughtful exchange that may occur over one or more visits.

Certain men are at substantially elevated risk for prostate cancer and may benefit from earlier and potentially more intensive screening discussions. African American men have the highest incidence and mortality rates from prostate cancer of any racial or ethnic group, with approximately sixty percent higher incidence and two to three times higher mortality compared to white men. Men with a first-degree relative diagnosed with prostate cancer have approximately double the risk. Carriers of BRCA1 and particularly BRCA2 mutations have significantly increased prostate cancer risk and tend to develop more aggressive disease. For these high-risk populations, professional organizations including the American Urological Association recommend initiating the screening discussion at age forty to forty-five rather than fifty-five. The informed consent process for these men should acknowledge their elevated risk while still presenting a balanced view of the benefits and limitations of PSA testing.

When PSA is elevated, the clinician must guide the patient through a nuanced evaluation process. A PSA level of four to ten nanograms per milliliter falls in the diagnostic gray zone, with approximately twenty-five percent of men in this range harboring prostate cancer on biopsy. The free PSA ratio can help refine risk assessment, as a lower percentage of free PSA suggests a higher probability of cancer. PSA density, calculated by dividing the PSA by the prostate volume estimated on ultrasound, accounts for the contribution of benign prostatic tissue. PSA levels exceeding ten nanograms per milliliter carry a substantially higher probability of cancer and generally prompt urological referral for biopsy consideration. Referral to urology for further evaluation, potentially including multiparametric MRI and targeted biopsy, is appropriate for men with elevated PSA in whom the decision to investigate has been made through shared discussion. Active surveillance, involving serial PSA monitoring, repeat biopsy, and clinical assessment, has emerged as the preferred management strategy for men diagnosed with low-risk prostate cancer, avoiding the morbidity of immediate treatment while maintaining the option for intervention if the cancer progresses.

Section 6: Testosterone Deficiency

Testosterone deficiency, also termed male hypogonadism, manifests through a constellation of symptoms spanning sexual, physical, and psychological domains. Sexual symptoms include decreased libido, erectile dysfunction, reduced frequency of spontaneous erections, and difficulty achieving orgasm. Physical symptoms include decreased muscle mass and strength, increased body fat particularly in the abdominal distribution, decreased bone mineral density with increased fracture risk, reduced energy and exercise tolerance, and decreased facial and body hair. Psychological symptoms include depressed mood, decreased motivation and drive, difficulty concentrating, and irritability. Additional findings may include normocytic anemia, gynecomastia, and small or soft testes on examination. The clinical presentation is often insidious and overlaps substantially with normal aging, depression, and other chronic conditions, making recognition challenging without a high index of suspicion.

Diagnosis of testosterone deficiency requires both clinical symptoms and biochemical confirmation with consistently low serum testosterone levels. Morning total testosterone is the initial test of choice, as testosterone levels exhibit a diurnal pattern with peak values occurring in the early morning hours, and samples obtained later in the day may be falsely low. A total testosterone level below three hundred nanograms per deciliter is generally accepted as the threshold for deficiency, though this value represents a consensus reference point rather than a precise physiological cutoff. Crucially, the diagnosis requires low levels on at least two separate morning measurements obtained on different days, as testosterone levels fluctuate significantly with acute illness, sleep deprivation, medication effects, and stress. When total testosterone is borderline, measurement of free testosterone or bioavailable testosterone provides additional diagnostic information, as conditions that alter sex hormone-binding globulin levels, including obesity, aging, and liver disease, affect total testosterone without necessarily reflecting the biologically active fraction. Measurement of luteinizing hormone and follicle-stimulating hormone distinguishes primary hypogonadism, in which the testes fail to produce adequate testosterone despite elevated gonadotropins, from secondary hypogonadism, in which hypothalamic or pituitary dysfunction results in inadequate gonadotropin stimulation.

Testosterone testing should be performed in men with specific symptoms suggestive of deficiency and is not recommended as a routine screening test in asymptomatic men. Appropriate indications for testing include the presence of characteristic symptoms such as persistent unexplained fatigue, low libido, or erectile dysfunction not responsive to PDE5 inhibitors. Certain medical conditions are associated with a higher prevalence of testosterone deficiency and warrant testing when symptoms are present, including HIV infection, chronic opioid therapy, type 2 diabetes, and obesity. The use of medications known to suppress testosterone, including chronic opioids, glucocorticoids, and certain antiepileptic drugs, should prompt evaluation in symptomatic men. Aging alone is not an indication for testosterone testing or treatment, as the modest decline in testosterone that occurs with normal aging does not necessarily warrant intervention and the benefits of treatment in older men with marginally low levels are unclear. The clinical context, including the patient's overall health status, comorbidities, and symptom burden, should guide the decision to test and to treat.

Testosterone replacement therapy is available through several delivery routes, each with distinct advantages and considerations. Topical testosterone gel, applied daily to the shoulders, upper arms, or abdomen, provides stable physiologic levels and is the most commonly prescribed formulation, though precautions must be taken to prevent transfer to women and children through skin contact. Intramuscular testosterone cypionate or enanthate, injected every one to two weeks, is effective and economical but produces supraphysiologic peaks and subtherapeutic troughs that may cause fluctuating symptoms and mood. Transdermal testosterone patches, applied daily, provide consistent levels but may cause local skin irritation. Contraindications to testosterone replacement include known or suspected prostate cancer, breast cancer in men, PSA greater than four nanograms per milliliter without urological evaluation, untreated severe obstructive sleep apnea, hematocrit greater than fifty percent, and uncontrolled heart failure. Monitoring during therapy includes hematocrit at three to six months and then annually, as testosterone stimulates erythropoiesis and polycythemia is the most common adverse effect, along with PSA measurement, symptom assessment, and lipid profile. Patients desiring future fertility should be counseled that exogenous testosterone suppresses spermatogenesis, often to azoospermia, and alternative treatments such as clomiphene citrate or human chorionic gonadotropin may be preferred in this population.

Section 7: Testicular and Scrotal Conditions

Testicular cancer is the most common solid malignancy in men aged fifteen to thirty-five years and, unlike most cancers, has an excellent prognosis with greater than ninety-five percent overall cure rates when detected and treated appropriately. The primary risk factor is a history of cryptorchidism, with the risk remaining elevated even after surgical orchiopexy, and family history of testicular cancer in a first-degree relative also confers increased risk. The classic presentation is a painless, hard testicular mass that does not transilluminate, distinguishing it from fluid-filled structures such as hydroceles. Any firm testicular mass should be considered malignant until proven otherwise, and the initial evaluation is urgent testicular ultrasound, which has high sensitivity and specificity for distinguishing intratesticular solid masses from benign extratesticular conditions. Urology referral should not be delayed, as radical inguinal orchiectomy is both diagnostic and therapeutic. Serum tumor markers including alpha-fetoprotein, human chorionic gonadotropin, and lactate dehydrogenase are obtained before surgery and are essential for staging and monitoring treatment response.

Epididymitis is the most common cause of acute scrotal pain in adult men and is characterized by gradual onset of unilateral scrotal pain and swelling. The etiologic agents differ by age group: in sexually active men under age thirty-five, Chlamydia trachomatis and Neisseria gonorrhoeae are the most common pathogens, while in men over thirty-five and those who practice insertive anal intercourse, enteric organisms, particularly Escherichia coli and other urinary tract pathogens, predominate. Physical examination reveals a tender, swollen epididymis, often with overlying erythematous scrotal skin, and a positive Prehn sign, defined as relief of pain with elevation of the affected testicle. Urinalysis and nucleic acid amplification testing for chlamydia and gonorrhea should be obtained. Treatment is directed at the suspected pathogen: ceftriaxone plus doxycycline for sexually transmitted epididymitis, and a fluoroquinolone such as levofloxacin for enteric organism-related disease.

Testicular torsion is a surgical emergency requiring urgent recognition and intervention to preserve testicular viability. It results from twisting of the spermatic cord, which compromises blood flow to the testis, and occurs most commonly in adolescent males though it can present at any age. The hallmark presentation is sudden onset of severe unilateral scrotal pain, often accompanied by nausea and vomiting, without preceding trauma or urinary symptoms. Physical examination reveals a high-riding testis with a horizontal lie, significant tenderness, and an absent cremasteric reflex on the affected side. The Prehn sign is characteristically negative, meaning that testicular elevation does not relieve pain, in contrast to epididymitis. While Doppler ultrasound can confirm absent testicular blood flow, clinical suspicion alone should prompt immediate urological consultation and surgical exploration without waiting for imaging, as the testicular salvage rate drops dramatically after six hours of ischemia, from approximately ninety percent to less than ten percent after twenty-four hours.

A varicocele is a dilation of the pampiniform venous plexus of the spermatic cord that presents as a painless scrotal mass with a characteristic bag-of-worms feel on palpation that becomes more prominent with standing and Valsalva maneuver. Approximately ninety percent of varicoceles occur on the left side due to the left testicular vein draining into the left renal vein at a right angle, creating higher venous pressure. Varicoceles are present in approximately fifteen percent of adult men and in up to forty percent of men evaluated for infertility, representing a significant and potentially treatable cause of male subfertility through mechanisms including elevated intrascrotal temperature and reflux of adrenal metabolites. Most varicoceles are asymptomatic and incidental, requiring no treatment. Surgical correction through varicocelectomy or radiologic embolization is considered for men with symptomatic varicoceles causing pain or discomfort and for men with documented infertility and abnormal semen parameters, as treatment has been shown to improve semen quality and pregnancy rates in appropriately selected patients.

Section 8: Sexual Health

Sexually transmitted infection screening in men should be integrated into routine primary care, with the frequency and scope of testing tailored to individual risk factors. HIV screening is recommended at least once for all adults aged thirteen to sixty-four, with annual testing for men at increased risk including men who have sex with men, injection drug users, and those with multiple sexual partners. Syphilis screening with RPR or VDRL is recommended for men who have sex with men, HIV-positive individuals, and those in other high-risk groups. Gonorrhea and chlamydia screening is recommended for sexually active men who have sex with men at all anatomic sites of exposure including urethral, pharyngeal, and rectal, with annual or more frequent testing depending on risk behavior. Hepatitis B screening should be performed in at-risk individuals, with vaccination offered to those without immunity. Universal hepatitis C screening is recommended once for all adults aged eighteen to seventy-nine, reflecting the high prevalence of undiagnosed chronic hepatitis C infection.

Healthcare for men who have sex with men requires specific clinical competencies and an affirming, nonjudgmental approach. STI screening in this population should be comprehensive and frequent, including testing at all anatomic sites of sexual contact (urethral, pharyngeal, and rectal) at least annually and more often for men with multiple partners. Pre-exposure prophylaxis for HIV prevention should be offered to all HIV-negative men who have sex with men who are at risk for HIV acquisition, including those with an HIV-positive partner, inconsistent condom use, or recent STI diagnosis. HPV vaccination is recommended through age twenty-six, with shared decision-making for ages twenty-seven to forty-five, and provides protection against anal cancer, which occurs at significantly elevated rates in men who have sex with men. Hepatitis A vaccination should be ensured given the increased transmission risk in this population. Inclusive care requires using nonjudgmental language, asking open-ended questions about sexual practices, and creating a clinical environment where patients feel safe disclosing their sexual identity and behaviors.

Discussing sexual health with male patients requires deliberate effort to normalize the conversation and reduce barriers to disclosure. Beginning with a framing statement such as "I ask all my patients about sexual health because it's an important part of overall wellness" communicates that the discussion is routine and nonjudgmental. Open-ended questions such as "Tell me about your sexual health" or "Are you currently sexually active?" allow the patient to guide the conversation initially. More specific follow-up questions should address the number and gender of sexual partners, type of sexual practices, consistency of protection, and any symptoms including discharge, genital lesions, or pain. The conversation should take place in a private setting with assurance of confidentiality, as many men are reluctant to initiate discussions about sexual concerns. The clinician should be comfortable asking about erectile function, ejaculatory concerns, and satisfaction, as these topics are often of significant concern to patients but are rarely raised spontaneously.

Premature ejaculation is the most common male sexual dysfunction, affecting approximately twenty to thirty percent of men, and is defined as ejaculation that consistently occurs within approximately one minute of vaginal penetration, is associated with a perceived inability to delay ejaculation, and causes personal distress or interpersonal difficulty. The condition is classified as lifelong, with onset from the first sexual experience, or acquired, with development after a period of normal ejaculatory function, with the latter potentially associated with erectile dysfunction, prostatitis, thyroid disease, or psychological factors. First-line treatment includes behavioral techniques such as the stop-start method and the squeeze technique, which involve learning to recognize and modulate pre-ejaculatory arousal. Pharmacologic treatment includes off-label use of SSRIs such as dapoxetine, sertraline, or paroxetine, which delay ejaculation through serotonergic modulation, and topical anesthetic agents such as lidocaine-prilocaine cream applied to the glans penis before intercourse. Referral to a sexual health specialist or sex therapist is appropriate for men with refractory symptoms or when significant relationship distress is present.

Section 9: Mental Health in Men

Mental health in men presents unique challenges rooted in the intersection of biology, psychology, and sociocultural expectations. Traditional masculinity norms that emphasize toughness, self-reliance, and emotional stoicism create significant barriers to acknowledging psychological distress and seeking help. Men are substantially less likely than women to be diagnosed with depression, yet their completed suicide rate is approximately four times higher, a paradox largely explained by the use of more lethal means, particularly firearms, and the tendency to present with symptoms that fall outside traditional diagnostic criteria. Men with mental health conditions are more likely to self-medicate with alcohol and other substances, which may mask underlying depression or anxiety while compounding the problem. The primary care physician occupies a critical position for identifying mental health conditions in men, as many men who die by suicide have visited a healthcare provider in the weeks or months before their death, representing missed opportunities for intervention.

Depression in men frequently presents differently from the classic diagnostic criteria and may be overlooked when clinicians rely solely on traditional symptom checklists. While sadness and anhedonia may be present, men with depression more commonly exhibit irritability, anger outbursts, and aggressive behavior as their predominant mood disturbance. Behavioral manifestations may include increased risk-taking, reckless driving, substance use, workaholism, and social withdrawal, rather than the tearfulness and emotional expressiveness more commonly associated with female presentations. Somatic complaints including chronic fatigue, diffuse pain, headaches, digestive disturbances, and sleep changes may be the presenting symptoms that bring men to the primary care office. The PHQ-2 and PHQ-9 remain valid screening instruments for men and should be administered routinely, but clinicians must also recognize atypical presentations and inquire about irritability, anger, and behavioral changes when the standard screening may not capture the full picture.

Suicide risk assessment in men requires specific attention to demographics, risk factors, and means access. The highest-risk demographic is white men over age seventy-five, though significant risk exists across all age groups, with military veterans, those with chronic pain or terminal illness, and men experiencing relationship dissolution or financial crisis being particularly vulnerable. Men are more likely to use lethal means, with firearms accounting for the majority of male suicide deaths, making discussion of safe firearm storage and voluntary temporary removal during crisis periods a potentially lifesaving intervention. Direct questioning about suicidal ideation should be routine when clinical suspicion exists, using clear language such as "Are you having thoughts of harming yourself or ending your life?" that normalizes the question and avoids ambiguity. The 988 Suicide and Crisis Lifeline provides immediate telephone and text-based support. Assessment should evaluate the presence of a specific plan, access to means, preparatory behaviors, and protective factors including social support and reasons for living.

Engaging men in mental healthcare requires adaptation of traditional therapeutic approaches to align with masculine values and communication styles. Normalizing mental health concerns by framing them in terms that resonate with men, such as "Many men experience this during stressful times" or "This is a common reaction to what you're going through," reduces perceived stigma. Functional framing that connects emotional health to performance, productivity, and relationships is often more motivating for men than discussions focused on emotions alone. Practical, action-oriented approaches, including specific behavioral activation steps, exercise prescriptions, and concrete problem-solving strategies, align with the preference many men have for doing rather than discussing. Privacy and confidentiality should be explicitly assured, as concerns about disclosure may inhibit help-seeking. Brief check-in conversations at routine visits, rather than formal mental health appointments, may provide a less stigmatizing entry point for men reluctant to seek dedicated mental health care. Collaborative care models that integrate behavioral health within the primary care setting have shown particular effectiveness in reaching men who would not otherwise access mental health services.

Section 10: Engaging Men in Healthcare

Multiple barriers contribute to men's lower utilization of preventive healthcare services, and understanding these barriers is essential for developing effective engagement strategies. Traditional masculinity norms that frame illness as weakness and self-sufficiency as paramount lead many men to delay seeking care until symptoms are severe or disabling. Work schedule conflicts, particularly for hourly workers without paid leave for medical appointments, represent a significant structural barrier. The healthcare system itself may be perceived as oriented toward women and children, with clinic environments, health promotion materials, and scheduling practices that do not adequately address men's needs and preferences. Lack of awareness about age-appropriate screening recommendations means that many men do not know what preventive services they should receive. Fear of diagnosis, procedures, and the implications of a positive finding also inhibits engagement, particularly for conditions such as prostate and colorectal cancer that involve examinations perceived as uncomfortable or emasculating.

Healthcare systems and practices can implement structural changes to improve men's engagement with preventive care. Extended and flexible scheduling including early morning, evening, and weekend appointments accommodates work schedules that constitute a primary barrier. Efficient visit design that respects time constraints and minimizes unnecessary waiting communicates respect for the patient's time and encourages return visits. Health messaging targeted toward men should use action-oriented, direct language and avoid overly emotional appeals that may conflict with masculine identity. Partner and family involvement can be leveraged, as women often serve as healthcare gatekeepers for their male partners and can encourage engagement. Opportunistic screening at every encounter, regardless of the presenting complaint, maximizes the yield of each visit and captures men who present infrequently, converting an acute care visit into a comprehensive health assessment.

Communication strategies tailored to men's preferences enhance therapeutic alliance and clinical effectiveness. Many men prefer direct, matter-of-fact communication that gets to the point quickly, so clinicians should present information concisely and prioritize actionable recommendations. Normalizing health concerns with statements such as "I see this commonly in men your age" reduces perceived stigma and isolation. Respecting patient autonomy through explicit shared decision-making acknowledges men's preference for control over their health decisions. Action-oriented counseling that provides specific, concrete steps rather than open-ended behavioral goals aligns with problem-solving orientations common among men. Follow-up plans should include clear next steps with defined timelines, as men who leave an encounter without a concrete plan may be less likely to follow through. Written or electronic after-visit summaries reinforce key messages and provide a reference for implementation.

Family medicine is uniquely positioned to address men's health needs through its core principles of relationship-based, comprehensive, longitudinal care. Building a therapeutic relationship over time creates the trust necessary for men to disclose sensitive concerns including sexual dysfunction, mental health symptoms, and substance use, topics that may not be raised at an initial encounter but emerge as rapport develops. Comprehensive care addressing the full spectrum of health needs, from preventive services through chronic disease management to acute concerns, allows the family physician to provide efficient, coordinated care in a single medical home. Proactive prevention strategies, including panel management with outreach to overdue patients and recall systems for screening, can engage men who would otherwise fall through the cracks. The family physician also serves as a coordinator and navigator, helping men access specialty care, navigate insurance barriers, and understand complex health information. Advocacy for men's health at the community and policy level, including support for workplace wellness programs, school-based health education, and public health campaigns targeting men, extends the physician's impact beyond the individual encounter.
