Health Equity and Disparities in Vascular Disease
Introduction
Disparities in vascular disease are evident across racial, ethnic, socioeconomic, and geographic lines, affecting prevalence, diagnosis, treatment, and outcomes. Health equity refers to the principle that every patient should have a fair opportunity to achieve optimal vascular health, without being disadvantaged due to their social position or identity. To effectively address these disparities, it is crucial first to understand their scope and underlying causes.
Scope of Vascular Disease Disparities
Racial and Ethnic Disparities
Peripheral arterial disease (PAD) is approximately twice as prevalent in Black Americans compared to White Americans, even after accounting for traditional risk factors. Black patients are also two to four times more likely to undergo major amputation than White patients with comparable disease severity. Hispanic and Native American populations experience higher rates of diabetes-related PAD and amputation. Although abdominal aortic aneurysm (AAA) is less common in Black and Hispanic populations, these groups tend to have worse outcomes following repair. In carotid disease, Black patients are less likely to receive carotid endarterectomy and more frequently present with stroke rather than asymptomatic disease. Across multiple vascular procedures, Black patients experience higher perioperative mortality and complication rates.
Socioeconomic Disparities
Individuals in lower socioeconomic strata exhibit higher rates of PAD, chronic limb-threatening ischemia (CLTI), and amputation. Insurance status plays a significant role; uninsured and Medicaid patients often present with more advanced disease and have higher amputation rates. These patients also face limited access to vascular specialists and noninvasive vascular laboratories, along with lower rates of preventive care, smoking cessation programs, and support for medication adherence.
Geographic Disparities
Rural populations frequently encounter limited access to vascular surgery expertise and advanced endovascular technology. The southeastern United States, known as the "amputation belt," has higher rates of major amputation, which correlate with poverty, diabetes prevalence, and restricted access to vascular specialists. Greater distance from a vascular center is associated with increased amputation rates and poorer outcomes. Telemedicine and hub-and-spoke models offer promising approaches to bridging these geographic gaps.
Sex and Gender Disparities
Women with PAD are more likely to be underdiagnosed and undertreated compared to men. They often present with atypical symptoms such as leg fatigue rather than classic claudication. Additionally, smaller vessel caliber in women may influence procedural outcomes. Women are underrepresented in vascular surgery clinical trials and have lower rates of AAA screening despite a significant disease burden.
Determinants of Vascular Health Disparities
Social Determinants of Health (SDOH)
Economic stability, including income, employment, food security, and housing stability, profoundly affects vascular health. Education influences health literacy, understanding of disease management, and the ability to navigate the healthcare system. Access to healthcare depends on insurance coverage, proximity to vascular specialists, and transportation availability. The neighborhood and built environment, encompassing walkability, safety, access to healthy food, and environmental exposures, also play critical roles. Social and community context, including social support, experiences of discrimination, and cultural factors, further shape health behaviors and outcomes.
Structural and Systemic Factors
Structural racism, manifested through historical and ongoing policies and practices, results in an inequitable distribution of resources, opportunities, and power. Residential segregation concentrates poverty, limits healthcare access, and increases exposure to environmental toxins. Implicit bias in clinical decision-making can unconsciously influence referral patterns, treatment recommendations, and pain management. Access to advanced endovascular technologies is often limited to centers of excellence located predominantly in affluent urban areas. Furthermore, clinical trial design and recruitment practices frequently exclude underrepresented populations.
Biologic and Genetic Factors
Higher prevalence of diabetes mellitus and hypertension among Black and Hispanic populations contributes to the burden of PAD. There may be genetic variations affecting susceptibility to atherosclerosis, vessel wall biology, and medication metabolism. However, biologic factors account for only a small portion of observed disparities, with social determinants being the dominant drivers.
The Amputation Disparity
The disparity in amputation rates is the most striking and consequential inequity in vascular surgery. Black patients undergo major amputation at two to four times the rate of White patients. This disparity arises from several factors: Black patients often present later with more advanced disease such as CLTI rather than claudication; they have lower rates of revascularization attempts before amputation; and they experience limited access to multidisciplinary limb preservation programs. Implicit bias may influence treatment recommendations, and a higher comorbidity burden, including diabetes and end-stage renal disease, complicates revascularization efforts. Limb preservation programs implemented in underserved communities have demonstrated significant reductions in amputation disparities.
Strategies for Advancing Health Equity
Expanding Access
Community-based PAD screening programs targeting underserved populations can utilize ankle-brachial index (ABI) measurement and risk factor education to identify disease earlier. Telemedicine and telehealth facilitate remote consultation, wound assessment, and follow-up care. Mobile vascular laboratories and point-of-care diagnostic testing can serve rural and underserved areas. Hub-and-spoke models connect community hospitals with vascular centers of excellence, while patient navigation services help address transportation barriers.
Workforce Diversity
Increasing the representation of underrepresented minorities in vascular surgery training and leadership enhances cultural competency and patient trust. Diverse surgical teams contribute to improved care delivery. Implicit bias training should be mandatory for all healthcare professionals involved in vascular care. Mentorship and pipeline programs are essential to attract underrepresented students to vascular surgery.
Quality Improvement and Accountability
Quality metrics should be stratified by race, ethnicity, sex, and socioeconomic status to identify disparities within institutions. Databases such as the Vascular Quality Initiative (VQI) and the National Surgical Quality Improvement Program (NSQIP) enable disparity tracking at both center and national levels. Institutions should set specific goals to reduce amputation disparities and close gaps in revascularization access. Reporting equity metrics alongside traditional quality indicators promotes accountability.
Community Engagement and Advocacy
Partnerships with community organizations, faith-based institutions, and public health agencies facilitate culturally tailored health education materials that address PAD risk factors and early warning signs. Advocacy efforts should focus on policies that expand insurance coverage, fund limb preservation programs, and address social determinants of health. Supporting research funding dedicated to health equity in vascular disease is also critical.
Research Priorities
Clinical trials in vascular surgery must include diverse populations and mandate reporting of outcomes by demographic subgroups. Research should evaluate the effectiveness of interventions designed to reduce disparities, such as community health workers, patient navigators, and telemedicine. Investigating the role of implicit bias in vascular surgical decision-making through standardized patient studies is important. Developing validated social risk screening tools for integration into vascular surgery practice will further advance equity.
Key Clinical Pearls
Racial disparities in amputation rates represent the most significant health equity issue in vascular surgery, driven primarily by social determinants and systemic factors rather than biologic differences. Every vascular surgery practice should stratify its quality metrics by race, ethnicity, and insurance status to identify local disparities. Implicit bias training must be mandatory for vascular surgery trainees and faculty. Expanding access to limb preservation programs in underserved communities stands out as the single most impactful intervention to reduce amputation disparities. Ultimately, health equity is not merely a social justice concern but a fundamental quality of care issue.
