Rationale for LAI Antipsychotics
The Adherence Problem in Psychotic Disorders
Non-adherence to oral antipsychotics affects 40-60% of patients with schizophrenia within the first year. Consequences of non-adherence: relapse, hospitalization, functional decline, increased mortality. Causes are multifactorial: lack of insight (anosognosia), side effects, cognitive impairment, substance use, complex regimens, stigma. LAIs remove the daily decision to take medication, providing guaranteed drug delivery.
Advantages of LAIs Over Oral Formulations
Transparent adherence monitoring -- missed injections are immediately apparent. Elimination of first-pass metabolism (more predictable plasma levels) Lower peak-to-trough plasma fluctuations (fewer side effects related to peak levels) Reduced relapse and rehospitalization rates (mirror-image and cohort studies) Early identification of non-adherence (missed appointment vs. covert oral non-adherence)
Available Formulations
First-Generation LAIs
Fluphenazine decanoate: 12.5-100 mg IM every 2-4 weeks; oldest available LAI; high EPS risk. Haloperidol decanoate: 50-450 mg IM every 4 weeks; dose conversion approximately 10-20x the oral daily dose; high potency with significant EPS/TD risk.
Second-Generation LAIs
Risperidone microspheres (Risperdal Consta): 12.5-50 mg IM every 2 weeks; requires refrigeration; 3-week release delay necessitates oral supplementation for first 3 weeks. Paliperidone palmitate monthly (Invega Sustenna): 39-234 mg IM every month; deltoid or gluteal injection; initiation with two loading doses (234 mg day 1 deltoid, 156 mg day 8 deltoid) Paliperidone palmitate 3-monthly (Invega Trinza): 273-819 mg IM every 3 months; requires 4 months of stable paliperidone palmitate monthly before switching. Paliperidone palmitate 6-monthly (Invega Hafyera): 1092 mg or 1560 mg IM every 6 months; requires prior stabilization on monthly formulation. Aripiprazole monohydrate (Abilify Maintena): 300-400 mg IM every month; gluteal or deltoid; requires 14 days of oral aripiprazole overlap. Aripiprazole lauroxil (Aristada): 441-1064 mg IM every 4-8 weeks (dose-dependent interval); gluteal only; nano-crystal dispersion formulation (Aristada Initio) allows single-injection start with oral aripiprazole for 21 days. Olanzapine pamoate (Zyprexa Relhyprev): 150-405 mg IM every 2-4 weeks; post-injection delirium/sedation syndrome (PDSS) risk (0.07% per injection); requires 3-hour observation period and REMS certification.

Initiation Protocols -- Key Points
Each LAI has a unique initiation strategy -- know the loading doses and oral overlap periods. Paliperidone palmitate monthly is the only LAI that does not require oral overlap (uses loading doses instead) Olanzapine pamoate requires REMS certification and a 3-hour monitoring period after every injection.
Pharmacokinetics
Absorption and Distribution
LAIs are injected as esters dissolved in oil or as aqueous microsphere/nanocrystal suspensions. Slow absorption from the injection site creates a depot effect. Time to steady state varies: 3-4 months for most monthly formulations. Missed doses have a "buffer" period depending on formulation pharmacokinetics -- important for clinical management of late injections.
Metabolism
Once absorbed, LAI formulations are metabolized identically to their oral counterparts. Paliperidone (9-OH-risperidone): primarily renal elimination; dose adjustment needed for renal impairment. Aripiprazole: CYP2D6 and CYP3A4 metabolism; adjust dose for CYP2D6 poor metabolizers or concomitant inhibitors.
Role in Early Psychosis
Shifting the Paradigm
Traditional approach reserved LAIs for chronic, non-adherent patients -- emerging evidence supports earlier use. First-episode psychosis patients have the most to gain from relapse prevention (each relapse worsens prognosis) Randomized trials (PRELAPSE, others) demonstrate significant relapse reduction with LAIs vs. oral medications in early psychosis. Patient acceptance of LAIs is higher than clinicians typically assume when presented in a non-coercive, shared decision-making framework.
Barriers to LAI Adoption
Clinician bias: associating LAIs with coercion or chronic illness. Health system barriers: injection infrastructure, cold chain for some formulations, cost. Patient factors: needle phobia, injection site discomfort, perceived loss of autonomy. Underutilization despite evidence: LAI prescribing rates remain low globally.

Clinical Pearls
When offering an LAI, frame it as a convenience and relapse-prevention tool rather than a response to non-adherence -- reduces stigma and improves acceptance. Paliperidone palmitate monthly is the most commonly used LAI due to its simple loading-dose initiation without oral overlap. The post-injection delirium/sedation syndrome (PDSS) with olanzapine pamoate is rare (~0.07%) but serious -- facilities must be REMS-certified and patients must be observed for 3 hours with every injection. Always confirm the patient is tolerant of the oral formulation before initiating the corresponding LAI. Know your diazepam equivalent conversions and oral-to-LAI dose conversions; these differ by formulation and are a common source of dosing errors. LAIs are not "forced treatment" by default -- many patients prefer them once they experience the stability and convenience.
