Vulvar Dermatoses and Vulvodynia

Overview

Vulvar skin conditions are common but frequently misdiagnosed and undertreated. Patients often delay seeking care due to embarrassment, and clinicians may lack familiarity with vulvar dermatologic findings. Many conditions can be diagnosed clinically based on their characteristic appearance, but biopsy is essential when the diagnosis is uncertain or malignancy is suspected. Most vulvar dermatoses are chronic conditions that require long-term management and follow-up.

Lichen Sclerosus

Epidemiology

Lichen sclerosus has a bimodal age distribution, primarily affecting prepubertal girls and postmenopausal women, though it can occur at any age. Prevalence is estimated at 1 in 70 to 1,000 women. The condition has autoimmune associations, most commonly thyroid disease, as well as vitiligo, alopecia areata, and pernicious anemia.

Clinical Features

Intense pruritus is the most common symptom. Patients may also report burning, dyspareunia, dysuria, and painful defecation when perianal involvement is present. On examination, the skin appears porcelain-white, thin, and wrinkled with a texture described as "cigarette paper." The distribution classically follows a figure-of-eight or hourglass pattern around the vulva and perianus. Secondary changes from scratching include ecchymoses, fissures, and erosions. Over time, architectural changes develop including labial resorption, clitoral phimosis (burying of the clitoris under adhesions), and introital stenosis. Perianal involvement occurs in 30 to 50% of patients. In prepubertal girls, the ecchymoses and bleeding associated with lichen sclerosus may mimic sexual abuse.

Diagnosis

Clinical diagnosis is often sufficient in classic cases. Biopsy is recommended when the diagnosis is uncertain, when the condition fails to respond to treatment, when hyperkeratotic, thickened, or suspicious areas are present (to rule out vulvar intraepithelial neoplasia or squamous cell carcinoma), and when any focal lesion does not respond to treatment. Histology shows epidermal atrophy, hyalinization of the dermis, and a lymphocytic band beneath the dermis.

Treatment

The first-line treatment is an ultra-potent topical corticosteroid, specifically clobetasol propionate 0.05% ointment (ointment is preferred over cream). Induction therapy is applied nightly for 4 to 6 weeks, followed by a taper to a maintenance regimen of 1 to 3 times weekly indefinitely. Lichen sclerosus is a lifelong disease requiring lifelong management. Response rates are 90 to 95%, with symptoms improving before the skin appearance normalizes. Second-line options for steroid-resistant cases include calcineurin inhibitors such as tacrolimus 0.1% ointment or pimecrolimus 1% cream. Surgery is reserved for architectural complications including lysis of clitoral adhesions and perineoplasty for introital stenosis. Topical vaginal estrogen should be added if concurrent genitourinary syndrome of menopause is present.

Malignancy Risk

There is a 4 to 6% lifetime risk of vulvar squamous cell carcinoma in patients with lichen sclerosus. This risk is reduced with consistent topical steroid maintenance therapy. Annual vulvar examination is required for life, and any new or changing lesion, especially hyperkeratotic or non-responsive areas, must be biopsied.

Lichen Planus

Types Affecting the Vulva

Erosive lichen planus is the most common vulvar form and the most challenging to manage. Papulosquamous lichen planus presents with the classic violaceous papules and Wickham striae on oral mucosa. Hypertrophic lichen planus manifests as thickened plaques.

Clinical Features (Erosive LP)

Erosive lichen planus presents with bright red, glazed erosions at the vaginal introitus (vestibule) and inner labia minora. Desquamative vaginitis with purulent discharge is characteristic. Patients experience severe dyspareunia and vulvar burning. Progressive scarring leads to vaginal synechiae, introital stenosis, and loss of vaginal depth. The oral mucosa is often involved, and examination of the mouth may reveal Wickham striae, erosions, or desquamative gingivitis. A critical distinguishing feature is that lichen planus involves the vagina, whereas lichen sclerosus rarely does.

Diagnosis

Biopsy is essential because of clinical overlap with lichen sclerosus, vulvar intraepithelial neoplasia, and pemphigoid. Histology shows a band-like lymphocytic infiltrate at the dermal-epidermal junction, degeneration of the basal layer, "saw-tooth" rete ridges, and civatte bodies.

Treatment

Ultra-potent topical corticosteroid (clobetasol 0.05% ointment) is used for vulvar disease. Vaginal involvement requires intravaginal corticosteroid, either hydrocortisone suppositories at 25 mg nightly or clobetasol applied intravaginally. Vaginal dilators are used to prevent or treat vaginal stenosis. Second-line therapies include tacrolimus ointment and systemic immunosuppressants such as methotrexate or mycophenolate mofetil for severe or refractory disease. Surgery may be needed for lysis of synechiae or vaginal reconstruction in cases of severe stenosis. Long-term follow-up is necessary because there is a 1 to 3% risk of vulvar squamous cell carcinoma, which is lower than the risk with lichen sclerosus but still requires surveillance.

Vulvar Dermatitis (Eczema)

Types

Contact dermatitis is the most common vulvar dermatosis. It may be irritant, caused by soaps, detergents, pads, panty liners, tight clothing, overwashing, or incontinence, or allergic, triggered by fragrances, preservatives, topical medications, or latex. Atopic dermatitis may also affect the vulva as part of the atopic triad of eczema, asthma, and allergic rhinitis.

Clinical Features

Pruritus is the dominant symptom. Acute disease presents with erythema, edema, and weeping, while chronic disease shows lichenification, scaling, and fissuring. The distribution follows the pattern of the offending contact agent or the typical atopic distribution.

Treatment

The most important step is identifying and eliminating the irritant or allergen. Vulvar hygiene measures include gentle cleansing with water or a soap-free wash, avoidance of douching and fragranced products, use of cotton underwear, and patting dry rather than rubbing. A medium-potency topical corticosteroid such as triamcinolone 0.1% ointment is applied twice daily for 2 to 4 weeks and then tapered. Petroleum jelly serves as a barrier emollient. Antihistamines such as hydroxyzine 25 mg at bedtime may help with nighttime pruritus. Unlike lichen sclerosus, long-term use of ultra-potent steroids should be avoided for dermatitis.

Vulvar Biopsy Indications

Biopsy should be performed when the diagnosis remains uncertain after clinical examination, when appropriate treatment fails after 6 to 12 weeks, when a pigmented lesion is present (to rule out melanoma), when a raised, thickened, or verrucous lesion is identified (to rule out vulvar intraepithelial neoplasia or squamous cell carcinoma), when an ulcer does not heal, and when any asymmetric or changing lesion appears in the setting of lichen sclerosus or lichen planus.

Biopsy Technique

A punch biopsy of 3 to 4 mm is performed under local anesthesia with lidocaine and epinephrine. The biopsy should include the border of the lesion and adjacent normal tissue. Multiple biopsies may be needed for multifocal disease. Hemostasis is achieved with silver nitrate or Monsel solution.

Vulvodynia

Definition

Vulvodynia is vulvar pain lasting 3 months or longer without an identifiable cause. It is a diagnosis of exclusion, meaning that infection, dermatosis, neoplasia, and neurologic conditions must be ruled out before the diagnosis is made.

Classification

Generalized vulvodynia involves diffuse, unprovoked pain affecting the entire vulva. Localized vulvodynia, also called vestibulodynia, is pain localized to the vestibule that may be provoked by touch, intercourse, or tampon insertion, or may be unprovoked, or a combination. Provoked vestibulodynia is the most common subtype, characterized by pain specifically with vestibular touch or pressure. The condition can be primary, present since the first attempt at vaginal penetration, or secondary, developing after a period of pain-free intercourse.

Pathophysiology

The pathophysiology is multifactorial. Peripheral sensitization involves increased vestibular nerve fiber density and altered nociceptor function. Mast cell activation and neurogenic inflammation contribute to local tissue changes. Central sensitization develops over time, amplifying pain signals. Pelvic floor hypertonicity is present in most patients and both contributes to and results from the pain cycle. Hormonal factors may play a role, as low-dose oral contraceptive pill use, especially with anti-androgenic progestins, has been associated with the condition, though this remains controversial. A history of recurrent candidiasis may serve as an initiating factor that triggers the cascade of sensitization.

Diagnosis

The history typically reveals burning, stinging, rawness, or soreness of the vulva that may be provoked or unprovoked. The cotton swab (Q-tip) test is the key diagnostic maneuver: systematic palpation of the vestibule with a moistened cotton swab is performed, and the test is positive if it reproduces the patient's pain at vestibular sites, especially at the 5 and 7 o'clock positions. The vulva may appear entirely normal on examination, though mild erythema of the vestibule may be present. Candidiasis must be excluded by culture, and dermatoses should be ruled out by biopsy if indicated. Sexually transmitted infections and atrophic vaginitis are also excluded. The pelvic floor should be assessed for hypertonicity and trigger points.

Treatment (Multimodal Approach)

Pelvic Floor Physical Therapy

Pelvic floor physical therapy is the first-line treatment for vestibulodynia. It addresses pelvic floor hypertonicity, trigger points, and muscle coordination through manual therapy, biofeedback, progressive relaxation, and dilator therapy. Approximately 60 to 80% of patients experience significant improvement.

Topical Therapies

Topical lidocaine 5% ointment can be applied to the vestibule 15 to 20 minutes before intercourse or nightly for desensitization. Compounded topical estrogen/testosterone cream may improve vestibular tissue health, though evidence is limited. Compounded topical gabapentin/amitriptyline cream has some evidence for localized pain reduction.

Systemic Medications

Amitriptyline or nortriptyline at 10 to 75 mg nightly provides a neuromodulatory effect and is the most commonly used systemic agent. Gabapentin at 300 to 3,600 mg daily and pregabalin at 75 to 300 mg twice daily are alternatives. Duloxetine or venlafaxine, both SNRIs, offer dual pain and mood benefit.

Psychological Support

Cognitive behavioral therapy addresses pain catastrophizing, fear of penetration, and relationship strain. Sex therapy may be helpful for secondary sexual dysfunction. Mindfulness-based approaches are additional options.

Interventional

Modified vestibulectomy involves surgical excision of the painful vestibular tissue with a vaginal advancement flap. It is reserved for provoked vestibulodynia refractory to conservative treatment and has a success rate of 60 to 90% symptom improvement, making it the most effective intervention for refractory provoked vestibulodynia. Botulinum toxin injection into pelvic floor muscles for hypertonicity is off-label but has emerging evidence. Trigger point injections with local anesthetic into pelvic floor trigger points are another option.

Clinical Pearls

Lichen sclerosus requires lifelong maintenance therapy with ultra-potent topical corticosteroids. Inadequate treatment increases the risk of vulvar squamous cell carcinoma, which has a 4 to 6% lifetime incidence in these patients.

Lichen planus involves the vagina while lichen sclerosus does not. A speculum exam should always be performed to assess for desquamative vaginitis and vaginal synechiae.

Contact dermatitis is the most common vulvar dermatosis. A detailed history of potential irritants and allergens should always be obtained before reaching for a biopsy.

Vulvodynia is a diagnosis of exclusion. Infection (specifically candida by culture), dermatoses, and atrophy must always be ruled out before making the diagnosis.

The Q-tip test is the key diagnostic maneuver for provoked vestibulodynia. Systematic palpation of the vestibule reproduces the patient's pain, particularly at the 5 and 7 o'clock positions.

Pelvic floor physical therapy is the first-line treatment for vestibulodynia and is effective in the majority of patients.

Any vulvar lesion that does not respond to appropriate treatment within 6 to 12 weeks should be biopsied. Early vulvar intraepithelial neoplasia and squamous cell carcinoma can mimic benign dermatoses.
