Approach to Joint Pain: Pediatric and Adult Differential Diagnosis

Introduction

Joint pain is among the most common presenting complaints across the lifespan. The differential diagnosis varies dramatically by age, with septic arthritis, reactive arthritis, and juvenile idiopathic arthritis (JIA) predominating in children, while osteoarthritis, gout, and rheumatoid arthritis are more common in adults. Med-peds physicians must systematically approach arthralgia and arthritis using a framework that accounts for age, acuity, joint distribution, and associated features.

Diagnostic Framework

Key Historical Features

Acuity: Acute (hours-days) vs. chronic (>6 weeks) Joint count: Monoarticular, oligoarticular (2-4 joints), polyarticular (>=5 joints) Pattern: Symmetric vs. asymmetric; migratory vs. additive. Inflammatory vs. mechanical: Morning stiffness >30 minutes, swelling, and improvement with activity suggest inflammation; pain with use and relief with rest suggest mechanical etiology. Associated symptoms: Fever, rash, diarrhea, urethritis, eye findings, weight loss.

Infants: Septic arthritis, osteomyelitis, non-accidental trauma, congenital hip dysplasia. Toddlers/school-age: Transient synovitis, septic arthritis, JIA, leukemia, Henoch-Schonlein purpura. Adolescents: Slipped capital femoral epiphysis, Osgood-Schlatter, gonococcal arthritis, reactive arthritis, JIA. Young adults: Rheumatoid arthritis, reactive arthritis, inflammatory bowel disease-associated arthritis, gonococcal arthritis. Middle-aged adults: Rheumatoid arthritis, gout, pseudogout, osteoarthritis. Older adults: Osteoarthritis, gout, pseudogout, calcium pyrophosphate disease, septic arthritis.

Pediatric-Specific Conditions

Septic Arthritis

Emergency: Requires urgent joint aspiration and empiric antibiotics. Most common organism: Staphylococcus aureus across all pediatric ages; Kingella kingae in children 6 months to 4 years. Kocher criteria (fever >38.5, non-weight-bearing, ESR >40, WBC >12,000) help stratify risk for septic hip. Joint fluid: WBC >50,000/mm3 with >75% PMNs is highly suggestive; culture confirms diagnosis.

Transient Synovitis

Most common cause of acute hip pain in children aged 3-10 years. Self-limited, typically post-viral; child appears well. Must be distinguished from septic arthritis; low-probability Kocher criteria reassuring. Resolves within 1-2 weeks with rest and NSAIDs.

Juvenile Idiopathic Arthritis (JIA)

Arthritis of unknown etiology in a child <16 years, persisting >=6 weeks. Subtypes: Oligoarticular (most common, 4 or fewer joints), polyarticular (RF+ or RF-), systemic (quotidian fevers, salmon-colored rash, serositis), enthesitis-related, psoriatic. Complications: Uveitis (particularly in ANA-positive oligoarticular JIA); requires routine slit-lamp screening. Treatment: NSAIDs, intra-articular corticosteroids, DMARDs (methotrexate), biologics (anti-TNF agents)

Malignancy Mimicking Arthritis

Leukemia may present with bone pain, joint swelling, and limping; nighttime pain, limb pain out of proportion, and cytopenias are red flags. Always obtain CBC with differential in children with unexplained joint pain and constitutional symptoms. Neuroblastoma and bone tumors can cause periarticular pain.

Adult-Specific Conditions

Osteoarthritis

Most common joint disorder in adults; affects >30 million Americans. Mechanical pain: Worse with activity, better with rest; minimal morning stiffness (<30 minutes) Commonly affects DIP joints (Heberden nodes), PIP joints (Bouchard nodes), first CMC, hips, and knees. Radiographic findings: Joint space narrowing, osteophytes, subchondral sclerosis, subchondral cysts. Management: Exercise, weight loss, topical and oral NSAIDs, intra-articular corticosteroids, joint replacement for severe disease.

Gout

Monosodium urate crystal deposition; most commonly affects the first MTP joint (podagra) Acute attacks: Sudden onset of severe pain, erythema, swelling; often nocturnal. Diagnosis: Needle-shaped, negatively birefringent crystals on polarized microscopy of joint aspirate. Acute treatment: NSAIDs, colchicine, corticosteroids; urate-lowering therapy (allopurinol, febuxostat) for recurrent attacks. Rare in children; when present, consider metabolic disorders (Lesch-Nyhan syndrome, glycogen storage diseases)

Rheumatoid Arthritis

Symmetric polyarthritis affecting small joints (MCP, PIP, wrists) Morning stiffness >1 hour; systemic symptoms (fatigue, weight loss) Serologic markers: RF and anti-CCP antibodies (anti-CCP more specific) Early DMARD therapy (methotrexate) with treat-to-target strategy improves outcomes.

Diagnostic Workup

Synovial fluid analysis: Essential for acute monoarthritis; distinguishes septic, inflammatory, and non-inflammatory causes. Non-inflammatory: WBC <2,000, clear, viscous. Inflammatory: WBC 2,000-50,000, translucent, reduced viscosity. Septic: WBC >50,000, opaque, very low viscosity. Laboratory studies: CBC, ESR, CRP, uric acid, RF, anti-CCP, ANA, HLA-B27 as indicated. Imaging: Radiographs first-line; ultrasound for synovitis and effusion detection; MRI for soft tissue and early erosive disease.

When to Refer

Suspected septic arthritis: Emergent orthopedic or pediatric orthopedic consultation. New diagnosis of JIA: Pediatric rheumatology referral and ophthalmology for uveitis screening. Inflammatory arthritis with systemic features: Rheumatology referral for DMARD initiation. Monoarthritis with diagnostic uncertainty: Consider synovial biopsy.

Clinical Pearls

Acute monoarthritis is septic arthritis until proven otherwise; joint aspiration should not be delayed. In children with joint pain and constitutional symptoms, always check a CBC to rule out leukemia. ANA-positive oligoarticular JIA carries high risk for silent anterior uveitis; regular slit-lamp examinations are mandatory. Gout in a young patient or child should prompt investigation for inborn errors of purine metabolism. Synovial fluid analysis is the most important diagnostic test in acute monoarthritis, guiding treatment before culture results return.
