Health Disparities and Anti-Racism in Family Medicine
Introduction
Health disparities are preventable differences in health outcomes that are closely linked to social, economic, and environmental disadvantage. Family medicine, with its commitment to comprehensive and continuous care for all populations, is uniquely positioned to address these disparities. Anti-racism in medicine requires moving beyond simply acknowledging that disparities exist to actively dismantling the structures that produce them.
Defining Key Concepts
Health disparities refer to differences in disease prevalence, outcomes, and access to care that disproportionately affect marginalized populations. Health equity is the attainment of the highest level of health for all people, which requires removing obstacles such as poverty, discrimination, and systemic exclusion. Social determinants of health (SDOH) are the conditions in which people are born, grow, live, work, and age that shape health outcomes, including housing, education, food security, transportation, and neighborhood environment. Structural racism describes the policies, institutions, and practices that create and reinforce racial inequity across generations, independent of any individual's intent. Anti-racism is the active opposition to racism through policy change, institutional reform, and individual practice.
The Evidence of Disparities
Racial and Ethnic Disparities
Black Americans have higher rates of hypertension, diabetes, maternal mortality, infant mortality, and premature cardiovascular death compared to White Americans. The Black maternal mortality rate is approximately 3 times that of White women in the United States. Hispanic and Indigenous populations face disproportionate rates of diabetes, obesity, and limited access to specialty care. Asian American subgroups have heterogeneous health profiles that are often masked when data are aggregated into a single category. Critically, these disparities persist after controlling for income, education, and insurance status, which implicates structural factors rather than individual behavior as the primary drivers.
Socioeconomic Disparities
Poverty is the single strongest predictor of poor health outcomes. Uninsured and underinsured patients receive less preventive care, experience delayed diagnoses, and have worse outcomes across virtually every disease category. Food insecurity affects approximately 10% of US households and is associated with poorly controlled chronic disease, particularly diabetes and cardiovascular conditions. Housing instability is associated with increased emergency department utilization and worse chronic disease management.
Geographic Disparities
Rural communities face provider shortages, limited specialty access, and higher rates of preventable death. Urban health deserts lack pharmacies, grocery stores, and primary care clinics in low-income neighborhoods. Broadband access disparities limit telehealth utilization in rural and low-income areas, creating an additional barrier to care that became particularly visible during the expansion of telemedicine.
Structural Racism in Medicine
Historical Context
The Tuskegee syphilis study, in which Black men with syphilis were deliberately left untreated for decades, is one of the most well-known examples of unethical research and has entrenched medical mistrust in Black communities. Forced sterilization programs targeting Indigenous, Black, and disabled populations represent another chapter of medical abuse. Redlining, a set of federal housing policies that created segregated neighborhoods, produced enduring health consequences through concentrated poverty, environmental exposures, and limited access to resources. Racial bias in clinical algorithms, such as race-based GFR equations that are now being revised, has embedded inequity directly into clinical decision-making tools.
Bias in Clinical Practice
Implicit bias refers to unconscious attitudes that affect clinical decision-making. Studies have demonstrated that implicit bias produces measurable differences in pain management, referral patterns, and communication quality based on patient race. Pain management disparities are particularly well documented: Black and Hispanic patients are less likely to receive adequate analgesia in emergency departments. Diagnostic delays affect minority populations as well, with conditions such as lupus, ADHD, and autism diagnosed later in these groups. Language barriers reduce quality of care, and the use of professional medical interpreters rather than family members or untrained staff improves outcomes and reduces errors.
Anti-Racism Strategies in Clinical Practice
Individual Level
At the individual level, clinicians should complete implicit bias training and engage in ongoing self-reflection about how their assumptions may influence care. Professional medical interpreters should be used for all encounters with patients who have limited English proficiency, rather than relying on family members or untrained staff. Clinical guidelines should be applied equitably, and race-based clinical adjustments that lack biological justification should be questioned. Listening to and believing patient-reported symptoms and experiences of discrimination is fundamental to building trust.
Practice and Institutional Level
At the practice level, screening for SDOH using validated tools such as PRAPARE (Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences) and AHC-HRSN (the CMS-developed Accountable Health Communities Health-Related Social Needs screening tool) allows clinics to connect patients to community resources. Diversifying the healthcare workforce matters because physician-patient racial concordance is associated with improved outcomes and patient satisfaction. Community health worker programs can bridge gaps between clinics and underserved communities. Clinic materials and signage should be available in languages spoken by the patient population. Health equity audits that examine disparities within a practice's own patient panel, such as A1c control stratified by race or cancer screening rates stratified by insurance status, can reveal actionable gaps.
Policy and Advocacy Level
At the policy level, family physicians should advocate for Medicaid expansion, universal health coverage, and adequate reimbursement for safety-net providers. Supporting policies that address upstream social determinants, including affordable housing, living wages, and educational equity, targets the root causes of health disparities. Community-based participatory research that centers the priorities of affected communities ensures that research efforts are relevant and respectful. Pipeline programs to increase representation of underrepresented minorities in medicine help build a workforce that reflects the diversity of the patient population.
Social Determinants of Health Screening
SDOH Screening Tools
The two most widely used SDOH screening tools are PRAPARE and AHC-HRSN. Screening should cover food insecurity (using the Hunger Vital Sign, a validated 2-question screen), housing instability, transportation barriers, interpersonal violence, and social isolation. SDOH findings should be documented in the EHR using ICD-10 Z codes, such as Z59.0 for homelessness and Z59.4 for food insecurity, which allows tracking and population-level analysis.
Connecting Patients to Resources
Maintaining a community resource directory, such as through findhelp.org or 211, enables efficient referrals. Integrating social workers, patient navigators, and community health workers into the primary care team strengthens the connection between clinical care and social support. Closed-loop referral systems that track whether patients successfully access referred resources are important for ensuring that screening translates into meaningful action.
Measuring Progress
Clinical quality metrics should be tracked with stratification by race, ethnicity, language, and insurance status to identify within-practice disparities. Regular data review allows practices to identify and address inequitable outcomes. Explicit health equity goals should be set as part of quality improvement programs. Participation in national initiatives such as the AMA Center for Health Equity and AAFP Health Equity initiatives provides frameworks and benchmarks for progress.
Key Clinical Pearls
Health disparities are driven by structural determinants, not inherent biological differences; race is a social construct, not a biological risk factor. Social determinants of health should be screened for at every patient encounter, because food insecurity, housing instability, and transportation barriers directly affect chronic disease management. Anti-racism requires action beyond awareness: advocating for policy change, diversifying the workforce, and auditing one's own practice for inequitable outcomes are concrete steps. Professional medical interpreters should be used for all clinical encounters with patients who have limited English proficiency, as this is both a legal requirement and a patient safety imperative.
