Skin Cancer Recognition: Dermoscopy Basics for the Family Physician

Introduction

Skin cancer is the most common malignancy worldwide, and family physicians are frequently the first to evaluate suspicious lesions. Early recognition dramatically improves outcomes, particularly for melanoma, where 5-year survival exceeds 99% for localized disease but drops below 35% for distant metastases. Dermoscopy enhances diagnostic accuracy by 20 to 30% compared to naked-eye examination alone.

Epidemiology and Risk Factors

Basal cell carcinoma (BCC) is the most common skin cancer, locally invasive but rarely metastasizing. Squamous cell carcinoma (SCC) is the second most common, with a metastatic potential of 2 to 5%. Melanoma accounts for approximately 1% of skin cancers but causes the majority of skin cancer deaths.

Key Risk Factors

Key risk factors include Fitzpatrick skin types I and II (fair skin, light eyes, red or blond hair), a history of blistering sunburns particularly in childhood, chronic UV exposure from occupational or recreational sources, and immunosuppression (organ transplant recipients have a 65- to 250-fold increased SCC risk). Family history of melanoma, the presence of atypical or dysplastic nevi, and personal history of prior skin cancer also elevate risk.

The ABCDE Criteria for Melanoma

The ABCDE criteria provide a systematic framework for evaluating melanocytic lesions. Asymmetry means one half does not match the other. Border irregularity refers to ragged, notched, or blurred edges. Color variation involves multiple shades of brown, black, red, white, or blue within the same lesion. Diameter greater than 6 mm is a traditional threshold, though melanomas can be smaller. Evolving describes any change in size, shape, color, or symptoms over time.

The Ugly Duckling Sign

The ugly duckling sign refers to a lesion that looks different from all surrounding nevi. This pattern recognition approach has high sensitivity and is particularly useful in patients with many moles.

Introduction to Dermoscopy

Equipment and Technique

A dermatoscope provides 10x magnification with polarized or non-polarized light. Polarized dermoscopy requires no contact fluid and visualizes deeper structures including vessels and collagen. Non-polarized dermoscopy requires immersion fluid and is better for superficial features such as milia-like cysts and comedo-like openings. A systematic approach evaluates the lesion's global pattern first, then local features.

Two-Step Dermoscopy Algorithm

The first step determines whether the lesion is melanocytic or non-melanocytic. Melanocytic features include pigment network, globules, streaks, and blue-white veil. Non-melanocytic features include leaf-like structures (suggesting BCC) and comedo-like openings (suggesting seborrheic keratosis). The second step, applied only to melanocytic lesions, assesses for malignancy using a structured method.

Benign Lesions

Seborrheic keratosis displays comedo-like openings, milia-like cysts, sharp demarcation, and a "stuck-on" appearance. Dermatofibroma shows a central white scar-like patch with a peripheral delicate pigment network. Blue nevus presents as homogeneous blue pigmentation without other structures.

Basal Cell Carcinoma

BCC characteristically shows arborizing (tree-like) vessels that are thick, branching, and bright red. Additional features include leaf-like structures, spoke-wheel areas, and blue-gray ovoid nests. Ulceration is seen in advanced lesions. Pigment network is notably absent.

Squamous Cell Carcinoma

SCC displays central keratin or white structureless areas along with hairpin or glomerular vessels. It often arises within actinic keratoses, which show a characteristic strawberry pattern with rosettes.

Melanoma

Melanoma demonstrates an atypical pigment network with irregular meshwork, thick lines, and irregular holes. A blue-white veil appears as confluent blue pigmentation with an overlying white ground-glass haze. Irregular streaks and pseudopods extend at the periphery. Regression structures include peppering (gray-blue dots) and white scar-like areas. Atypical vascular patterns with dotted and linear-irregular vessels are additional clues.

Biopsy Techniques

Excisional biopsy is preferred for suspected melanoma and should include 1 to 3 mm margins. Shave biopsy is appropriate for suspected BCC or SCC but should be avoided for melanocytic lesions when possible. Punch biopsy is useful for larger lesions where excision is impractical, targeting the most atypical area. Specimens should always be submitted with a clinical description and differential diagnosis to guide pathology interpretation.

Screening Recommendations

There is no universal screening recommendation from the USPSTF, which issued an insufficient evidence (I) statement. Targeted screening is recommended for high-risk individuals. Skin self-examination and sun protection counseling should be encouraged for all patients. Full-body skin exams should be performed in patients with risk factors or concerning history.

Key Clinical Pearls

Dermoscopy increases melanoma sensitivity from approximately 60% to 90% compared to naked-eye examination, making it a skill worth investing time in learning. The ugly duckling sign is one of the most practical clinical tools: any lesion that looks different from its neighbors deserves closer evaluation. When in doubt, biopsy is the correct choice because a missed melanoma has far greater consequences than a benign biopsy. Organ transplant recipients need annual full-body skin exams due to dramatically elevated SCC risk. Dermoscopic findings should always be documented, and tele-dermoscopy should be considered for remote consultation when available.
