Obesity Prevention and Management in Primary Care
Overview
Obesity, defined as a BMI of 30 kg/m2 or higher, affects over 40% of US adults and is a major driver of type 2 diabetes, cardiovascular disease, certain cancers, and premature mortality. It is increasingly recognized as a chronic, relapsing disease with complex biological underpinnings, not simply a failure of willpower. Family physicians play a central role in prevention, screening, and multimodal management encompassing behavioral counseling, pharmacotherapy, and surgical referral when appropriate.
Pathophysiology
The traditional energy balance model, which frames obesity as the result of chronic caloric surplus, captures only part of the picture. The hypothalamus actively regulates body weight around a set point, and after weight loss, the body mounts a physiological counterattack: resting metabolic rate drops and appetite hormones shift to drive weight regain. Leptin resistance impairs satiety signaling, while ghrelin levels rise after weight loss to stimulate hunger. Insulin signaling, the gut microbiome's influence on energy extraction, and chronic low-grade inflammation all contribute to the metabolic environment that favors weight gain. Genetic factors are substantial, accounting for 40 to 70% of BMI variation across populations, though this is polygenic rather than driven by single genes. Environmental contributors, including the ubiquity of ultra-processed foods, sedentary built environments, chronic sleep deprivation, and sustained psychological stress, create the conditions in which genetic susceptibility is expressed.
Screening and Diagnosis
BMI Classification
BMI provides a standardized, population-level classification: underweight (below 18.5), normal weight (18.5 to 24.9), overweight (25 to 29.9), obesity class I (30 to 34.9), obesity class II (35 to 39.9), and obesity class III (40 and above).

Limitations of BMI
BMI has well-known limitations. It does not distinguish fat from lean mass, so muscular individuals may be misclassified. It does not capture fat distribution, which matters because visceral fat is far more metabolically dangerous than subcutaneous fat. Different thresholds may apply for Asian populations, for whom overweight is defined at a BMI of 23 or above and obesity at 27.5 or above. Adding waist circumference measurement provides prognostic value: measurements exceeding 40 inches in men or 35 inches in women indicate increased cardiometabolic risk.
Assessment
A thorough assessment includes the patient's weight trajectory over time, prior weight loss attempts and their outcomes, current medications that may promote weight gain, and the presence of obesity-related comorbidities. Medications commonly associated with weight gain include insulin, sulfonylureas, thiazolidinediones, corticosteroids, certain antidepressants (particularly mirtazapine and paroxetine), antipsychotics (olanzapine and quetiapine), beta-blockers, and gabapentin or pregabalin. Secondary causes such as hypothyroidism, Cushing syndrome, and polycystic ovary syndrome should be considered and screened for when clinically appropriate. Readiness for change should be assessed using motivational interviewing techniques, and clinicians should screen for disordered eating behaviors that might require specialized treatment.
Behavioral and Lifestyle Interventions
Dietary Approaches
Effective dietary intervention targets a caloric deficit of 500 to 750 kcal per day, aiming for a weight loss of 1 to 1.5 pounds per week. No single dietary pattern has been shown to be clearly superior to others; adherence is the strongest predictor of success. Evidence-based patterns include the Mediterranean diet, DASH diet, low-carbohydrate approaches, low-fat diets, and time-restricted eating. The common thread across effective diets is an emphasis on whole foods, increased fiber and protein, and reduction of ultra-processed foods. Referral to a registered dietitian for medical nutrition therapy is valuable and is covered by Medicare for patients with diabetes or chronic kidney disease.
Physical Activity
At minimum, 150 minutes per week of moderate-intensity aerobic activity is recommended. For weight loss maintenance, 200 to 300 minutes per week may be necessary. Resistance training preserves lean mass during caloric restriction, which is critical because muscle loss contributes to metabolic adaptation. Clinicians should emphasize the health benefits of regular movement independent of weight loss, as even modest activity improvements reduce cardiovascular risk, and encourage reductions in sedentary behavior.
Behavioral Counseling
The USPSTF recommends intensive, multicomponent behavioral interventions for adults with a BMI of 30 or above, defined as 12 or more sessions in the first year. Effective programs incorporate self-monitoring through food diaries and regular weigh-ins, goal setting, stimulus control, problem-solving strategies, and relapse prevention planning. The Diabetes Prevention Program (DPP) is an excellent referral option for patients with prediabetes and a BMI of 25 or above, offering a structured curriculum with proven efficacy.
Pharmacotherapy
General Principles
Pharmacotherapy should be considered when BMI is 30 or above, or when BMI is 27 or above with at least one weight-related comorbidity. Medications are an adjunct to lifestyle modifications, not a substitute. Depending on the agent, patients can expect 5 to 15% total body weight loss. The standard benchmark is to continue the medication if the patient achieves at least 5% weight loss at three months and to discontinue if the response is inadequate. Chronic therapy is typically needed, as weight regain occurs reliably after discontinuation of most anti-obesity medications.
GLP-1 Receptor Agonists
GLP-1 receptor agonists have transformed obesity medicine. Semaglutide (Wegovy), given as a 2.4 mg weekly subcutaneous injection, produces approximately 15 to 17% weight loss as demonstrated in the STEP trials. Liraglutide (Saxenda), at 3.0 mg daily subcutaneously, achieves approximately 8% weight loss. Tirzepatide (Zepbound), a dual GIP/GLP-1 receptor agonist, has produced the most dramatic results, with approximately 20 to 22% weight loss in the SURMOUNT trials. Common side effects include dose-dependent nausea, vomiting, and diarrhea that typically improve over time. Rare but serious risks include pancreatitis and gallstones. These agents are contraindicated in patients with a personal or family history of medullary thyroid carcinoma or MEN2. The SELECT trial demonstrated cardiovascular benefit for semaglutide independent of its weight-loss effects.
Other Approved Agents
Phentermine-topiramate ER (Qsymia) achieves approximately 10% weight loss but is contraindicated in pregnancy, glaucoma, and hyperthyroidism. Naltrexone-bupropion (Contrave) produces approximately 6% weight loss and should be avoided in patients using opioids or with a history of seizures. Orlistat (Xenical/Alli) offers approximately 5% weight loss but is limited by gastrointestinal side effects and fat-soluble vitamin malabsorption. Phentermine is approved for short-term use (up to 12 weeks per the FDA label, though some clinical guidelines support longer use) and requires cardiovascular monitoring given its sympathomimetic properties.

Emerging Agents
The pipeline for obesity pharmacotherapy is active. Higher-dose oral semaglutide formulations are in development for obesity indications. Retatrutide, a triple agonist targeting GLP-1, GIP, and glucagon receptors, has shown impressive early results. CagriSema combines semaglutide with cagrilintide, an amylin analog. Survodutide is a dual GLP-1/glucagon receptor agonist under investigation.
Bariatric Surgery
Indications
Traditional indications for bariatric surgery include a BMI of 40 or above, or a BMI of 35 or above with obesity-related comorbidities such as diabetes, hypertension, or obstructive sleep apnea. Updated 2022 guidelines from the ASMBS and IFSO have expanded consideration to patients with a BMI of 30 or above who have metabolic disease. Surgery is generally reserved for patients who have not achieved sustained weight loss with behavioral and pharmacologic approaches.
Procedures
Sleeve gastrectomy is now the most commonly performed bariatric procedure, producing 20 to 30% total body weight loss. Roux-en-Y gastric bypass (RYGB) achieves 25 to 35% weight loss and remains the gold standard for metabolic improvement, particularly diabetes remission. Adjustable gastric banding has declined in popularity due to lower efficacy and higher revision rates. The duodenal switch and SADI-S procedures produce the greatest weight loss but carry the highest nutritional risk.
Primary Care Follow-Up
Post-bariatric patients require lifelong follow-up. Vitamin and mineral supplementation (including B12, iron, calcium, vitamin D, and folate) is mandatory. Clinicians should monitor for dumping syndrome, nutritional deficiencies, and gallstone formation. Sleeve gastrectomy can worsen gastroesophageal reflux disease. Mental health support is important, as some patients develop or unmask substance use disorders after surgery. Contraception counseling is essential because fertility often improves after significant weight loss, and pregnancy should be avoided for 12 to 18 months post-operatively to allow nutritional stabilization.

Clinical Pearls
Framing obesity as a chronic disease rather than a lifestyle choice reduces stigma and improves patient engagement and treatment adherence. Person-first language matters: say "patient with obesity" rather than "obese patient." Asking permission before discussing weight ("Is it okay if we talk about your weight today?") respects patient autonomy and sets a collaborative tone. A 5 to 10% weight loss, though seemingly modest, produces clinically meaningful improvements in blood pressure, glucose, lipid profiles, and sleep apnea severity. Metabolic adaptation after weight loss, which reduces basal metabolic rate by approximately 15%, explains the near-universal tendency toward weight regain and supports the rationale for chronic pharmacotherapy. GLP-1 agonists have been transformative, but access barriers including cost, insurance coverage limitations, and supply shortages remain significant. Every medication list should be reviewed for weight-promoting drugs, with weight-neutral or weight-favorable alternatives substituted where possible (for example, metformin instead of sulfonylurea, or bupropion instead of mirtazapine). Weight cycling, or "yo-yo dieting," may be independently harmful, making sustained modest weight loss preferable to repeated dramatic fluctuations.
