Dermatitis Herpetiformis and Linear IgA Bullous Dermatosis
Introduction
Dermatitis herpetiformis (DH) and linear IgA bullous dermatosis (LABD) are autoimmune subepidermal blistering diseases distinguished by their immunofluorescence patterns. Although both feature IgA deposition at the dermoepidermal junction, they differ fundamentally in pathogenesis, clinical presentation, and management. Understanding these distinctions is essential for accurate diagnosis and targeted therapy.
Dermatitis Herpetiformis
Epidemiology and Pathogenesis
Dermatitis herpetiformis has a prevalence of 10 to 75 per 100,000 in Northern European populations and is strongly associated with HLA-DQ2 (present in over 90% of patients) and HLA-DQ8. The disease is driven by IgA antibodies against epidermal transglutaminase (eTG), the autoantigen in the skin, which cross-reacts with tissue transglutaminase (tTG) in the gut. Virtually all DH patients have an underlying celiac disease (gluten-sensitive enteropathy), although the majority are asymptomatic from a gastrointestinal standpoint. The pathogenic sequence begins with gluten ingestion, which triggers IgA anti-tTG production. IgA-eTG immune complexes then deposit in the dermal papillae, initiating the cutaneous disease.
Clinical Features
The eruption consists of intensely pruritic, grouped (herpetiform) papulovesicles distributed symmetrically on extensor surfaces, with a predilection for the elbows, knees, buttocks, posterior scalp, and upper back. The vesicles are often excoriated before clinical examination, so what the clinician typically encounters is erosions, crusts, and post-inflammatory changes rather than intact vesicles. The distribution is remarkably symmetric. Oral mucosal involvement is rare, which contrasts with LABD. Patients face an increased risk of intestinal lymphoma if gluten exposure remains uncontrolled.
Diagnosis
Histopathology reveals neutrophilic microabscesses at the dermal papillae with subepidermal cleavage, and eosinophils may also be present. The gold standard for diagnosis is direct immunofluorescence, which shows granular IgA deposits at the dermal papillary tips. Serologic testing for IgA anti-tTG and IgA anti-endomysial antibodies correlates with disease activity and gluten exposure. Small bowel biopsy demonstrates villous atrophy in most patients, though severity varies.

Management
Dapsone at an initial dose of 25 to 50 mg/day, titrated to 100 to 200 mg/day, provides rapid symptomatic relief within 24 to 48 hours. Monitoring for dapsone adverse effects is essential, including methemoglobinemia, hemolytic anemia (especially in G6PD deficiency), agranulocytosis, and dapsone hypersensitivity syndrome. The definitive long-term treatment is a strict gluten-free diet (GFD), which reduces the dapsone requirement over 6 to 24 months and decreases lymphoma risk. Sulfapyridine or sulfasalazine may be used in patients who cannot tolerate dapsone.
Linear IgA Bullous Dermatosis
Epidemiology and Pathogenesis
LABD is less common than DH and shows a bimodal age distribution, with a childhood form known as chronic bullous disease of childhood and a separate adult form. Drug-induced LABD is well recognized, with vancomycin being the most common culprit, followed by NSAIDs, lithium, and certain antibiotics. Autoantibodies target components of the basement membrane zone, most commonly the 120-kDa ectodomain of BP180 (LAD-1) and the 97-kDa antigen (LABD97). There is no association with celiac disease or gluten sensitivity.
Clinical Features
LABD presents with tense vesicles and bullae, often arranged in an annular or "cluster of jewels" pattern that is quite distinctive. In adults, lesions favor the trunk, groin, and lower extremities. Mucosal involvement is common, occurring in 50 to 70% of cases, and may include oral, ocular, and genital mucosa, sometimes mimicking cicatricial pemphigoid. In children, a perioral and perineal distribution is characteristic. Drug-induced LABD may present acutely with widespread bullae that can mimic toxic epidermal necrolysis.

Diagnosis
Histopathology shows a subepidermal blister with a predominantly neutrophilic infiltrate and may be indistinguishable from DH on H&E alone. The defining diagnostic feature is DIF showing linear IgA deposits along the BMZ, which separates LABD from DH. Indirect immunofluorescence on salt-split skin demonstrates IgA binding to the epidermal side (more commonly) or dermal side, depending on the target antigen. IgA anti-endomysial and anti-tTG antibodies are absent, further distinguishing it from DH.
Management
Dapsone is first-line therapy at dosing similar to DH. Colchicine at 0.6 mg two to three times daily is an alternative for dapsone-intolerant patients. For drug-induced LABD, withdrawal of the offending agent leads to resolution within days to weeks. Refractory cases may require mycophenolate mofetil, IVIg, or rituximab. Ocular involvement warrants prompt ophthalmology referral to prevent cicatricial complications.

Key Clinical Pearls
DIF is mandatory to distinguish DH from LABD because histopathology alone cannot reliably differentiate them. All DH patients should be screened for celiac disease and counseled on a strict gluten-free diet regardless of gastrointestinal symptoms. Dapsone provides rapid relief in both conditions but does not alter the underlying disease course in DH; only the gluten-free diet achieves that. Vancomycin-induced LABD should always be considered in hospitalized patients who develop acute blistering. G6PD testing is required before initiating dapsone therapy.
