Psoriasis: From Pathogenesis to Biologics
Overview
Psoriasis is a chronic, immune-mediated inflammatory disease affecting approximately 2 to 3 percent of the global population. Characterized by well-demarcated, erythematous, scaly plaques, it is now understood as a systemic disease with significant comorbidities including psoriatic arthritis, cardiovascular disease, and metabolic syndrome.
Epidemiology
Psoriasis shows a bimodal age of onset, with peaks at 20 to 30 years and 50 to 60 years. It affects both sexes equally, with higher prevalence in Caucasians and lower prevalence in East Asians and West Africans. HLA-Cw6 is the strongest genetic susceptibility locus, associated with early-onset type I psoriasis. Concordance is approximately 70 percent in monozygotic twins and 20 percent in dizygotic twins. About 30 percent of patients will develop psoriatic arthritis.
Pathogenesis
The IL-23/Th17 Axis
The central pathway in psoriasis pathogenesis involves dendritic cells and macrophages producing IL-23 in response to triggers, which sustains and expands Th17 cells. Th17 cells produce IL-17A and IL-17F (which stimulate keratinocyte proliferation, neutrophil recruitment, and antimicrobial peptide production) and IL-22 (which drives epidermal hyperplasia and acanthosis). Tissue-resident memory T cells (TRM) in previously affected skin explain why psoriasis tends to relapse at the same sites.
TNF-alpha Contribution
TNF-alpha, produced by dendritic cells, macrophages, and T cells, amplifies inflammation, promotes angiogenesis, and synergizes with IL-17. It was the target of first-generation biologics including infliximab, adalimumab, and etanercept.
Innate Immune Activation
Cathelicidin (LL-37) complexes with self-DNA to activate plasmacytoid dendritic cells via TLR9, triggering type I interferon production that amplifies the adaptive immune response. Keratinocytes act as active immune participants, producing chemokines and cytokines that sustain the inflammatory cycle.
Genetic Susceptibility
The PSORS1 locus on chromosome 6p21, which contains HLA-Cw6, accounts for approximately 50 percent of genetic risk. Other important loci include IL-23R, IL-12B, TNFAIP3, CARD14, and TRAF3IP2. CARD14 gain-of-function mutations can cause familial psoriasis via NF-kB activation.
Environmental Triggers
Streptococcal pharyngitis is the classic trigger for guttate psoriasis. Medications that can trigger or worsen psoriasis include lithium, beta-blockers, antimalarials, ACE inhibitors, IFN-alpha, and rapid corticosteroid withdrawal. Smoking and obesity increase risk in a dose-dependent manner. Psychological stress and the Koebner phenomenon (new lesions at sites of skin trauma) are additional triggers.
Clinical Subtypes
Chronic Plaque Psoriasis (Psoriasis Vulgaris)
This is the most common form, accounting for 80 to 90 percent of cases. It presents as well-demarcated, symmetric, erythematous plaques with silvery-white scale, favoring the elbows, knees, scalp, lumbosacral area, and umbilicus. The Auspitz sign — pinpoint bleeding upon removal of scale, reflecting exposed papillary dermal capillaries — is characteristic. The Woronoff ring, a blanching halo around resolving plaques, may also be seen.
Guttate Psoriasis
Small (1 to 10 mm) drop-like papules and plaques appear, often 2 to 3 weeks after streptococcal pharyngitis. It is common in children and young adults and may resolve spontaneously or progress to chronic plaque psoriasis. ASO titer and throat culture should be checked.
Inverse (Flexural) Psoriasis
This involves intertriginous areas — axillae, groin, inframammary folds, and the intergluteal cleft — presenting as shiny, erythematous plaques with minimal scale due to the maceration effect of these moist environments. The differential includes candidiasis, tinea, intertrigo, and seborrheic dermatitis.
Erythrodermic Psoriasis
Generalized erythema involving over 90 percent BSA constitutes a dermatologic emergency with risks of hypothermia, high-output cardiac failure, and infection. It may be triggered by rapid corticosteroid withdrawal, infection, or drug reaction and requires inpatient management.
Pustular Psoriasis
Generalized pustular psoriasis (von Zumbusch type) presents with sheets of sterile pustules on erythematous skin accompanied by fever and leukocytosis, often associated with IL-36RN mutations. Localized forms include palmoplantar pustulosis (debated as a distinct entity) and acrodermatitis continua of Hallopeau (sterile pustules of the digits and nails).
Nail Psoriasis
Nail involvement affects up to 50 percent of psoriasis patients and 80 percent of those with psoriatic arthritis. Nail matrix involvement produces pitting, leukonychia, red spots in the lunula, and crumbling. Nail bed involvement produces oil-drop discoloration, onycholysis, subungual hyperkeratosis, and splinter hemorrhages. Nail psoriasis predicts risk of psoriatic arthritis.
Scalp Psoriasis
Thick, adherent scale extending beyond the hairline is characteristic. Differentiating from seborrheic dermatitis (and the overlap entity sebopsoriasis) can be challenging. Scalp psoriasis can cause temporary hair loss.
Psoriatic Arthritis (PsA)
PsA develops in approximately 30 percent of psoriasis patients and may precede skin disease in about 15 percent. The CASPAR criteria are used for classification. Clinical patterns include distal interphalangeal predominant, oligoarticular, polyarticular (rheumatoid-like), spondylitis, and arthritis mutilans. Dactylitis ("sausage digits") and enthesitis are characteristic features. Screening tools such as the PEST and PASE questionnaires should be used at every visit.
Comorbidities
Psoriasis carries increased risk of cardiovascular disease (MI, stroke) and should be treated as an independent cardiovascular risk factor. Metabolic syndrome — obesity, dyslipidemia, insulin resistance, and NAFLD — is common. Depression and anxiety are prevalent, with DLQI scores often exceeding 10. Inflammatory bowel disease (especially Crohn disease) and uveitis (particularly with PsA or spondylitis) are additional associations.
Histopathology
Classic histologic features include regular acanthosis with elongation of rete ridges, suprapapillary thinning of the epidermis, confluent parakeratosis, loss of the granular layer, Munro microabscesses (neutrophils in the parakeratotic stratum corneum), spongiform pustules of Kogoj (neutrophils in the spinous layer), dilated and tortuous capillaries in the dermal papillae, and a perivascular lymphocytic infiltrate.
Severity Assessment
Mild disease involves less than 3 percent BSA with limited quality-of-life impact. Moderate disease involves 3 to 10 percent BSA or significant functional impairment. Severe disease involves over 10 percent BSA or any BSA with significant quality-of-life impact. PASI (Psoriasis Area and Severity Index) is the gold standard in clinical trials. The "rule of tens" serves as a quick guide: BSA over 10 percent, PASI over 10, or DLQI over 10 indicates moderate-to-severe disease.
Management
Topical Therapies (Mild Disease)
Topical corticosteroids are first-line, using medium-to-high potency for the body and low potency for the face and flexures. Vitamin D analogues such as calcipotriol are often combined with betamethasone dipropionate. Tazarotene (a topical retinoid) is effective but irritating. Calcineurin inhibitors are used off-label for facial and flexural psoriasis. Tapinarof (AhR agonist) offers a steroid-free option.
Phototherapy (Moderate Disease)
Narrowband UVB (311 nm) is the first-line phototherapy at 2 to 3 times per week. PUVA is reserved for recalcitrant disease but carries higher carcinogenicity risk. Excimer laser (308 nm) provides targeted phototherapy for localized plaques.
Conventional Systemic Therapies
Methotrexate is given weekly (oral or subcutaneous) with CBC and LFT monitoring and folic acid supplementation; it is effective for both skin and joint disease. Cyclosporine has rapid onset but is limited to intermittent short courses (6 to 12 months) due to nephrotoxicity and hypertension. Acitretin, a retinoid, is useful for pustular psoriasis and as an adjunct to phototherapy, but is teratogenic — women of childbearing potential must avoid pregnancy for 3 years after discontinuation. Apremilast (PDE4 inhibitor) has modest efficacy with GI side effects, weight loss, and requires depression monitoring.
Biologic Therapies
TNF-alpha inhibitors (etanercept, infliximab, adalimumab, certolizumab pegol) were the first-generation biologics, effective but generally less so than IL-17 and IL-23 agents. They carry risks of serious infection and TB reactivation (screen before starting).

IL-17 inhibitors include secukinumab (anti-IL-17A, PASI 90 approximately 60 to 70 percent), ixekizumab (anti-IL-17A, slightly faster onset, PASI 90 approximately 70 percent), bimekizumab (anti-IL-17A and IL-17F, achieving the highest PASI 100 rates at 30 to 40 percent), and brodalumab (anti-IL-17RA, effective but carrying a boxed warning for suicidal ideation). Adverse effects include oral and genital candidiasis, and caution is needed in patients with IBD, as IL-17 blockade can exacerbate Crohn disease.
IL-23 inhibitors include guselkumab (anti-IL-23 p19, every 8 weeks after loading), risankizumab (anti-IL-23 p19, every 12 weeks, PASI 90 approximately 70 to 75 percent), and tildrakizumab (anti-IL-23 p19, every 12 weeks, slightly lower efficacy). IL-23 inhibitors have the best durability of response and may induce "treatment-free remission" in some patients, with a favorable safety profile requiring minimal monitoring.
Ustekinumab (anti-IL-12/23 p40), given every 12 weeks with weight-based dosing, is effective for skin and joint disease with over 10 years of long-term safety data, but is being supplanted by more selective IL-23 p19 agents.
Treatment of Psoriatic Arthritis
Options include TNF inhibitors (adalimumab, etanercept, infliximab, certolizumab, golimumab), IL-17 inhibitors (secukinumab, ixekizumab), JAK inhibitors (tofacitinib, upadacitinib), IL-23 inhibitors (guselkumab, FDA-approved for PsA), apremilast for mild PsA, and methotrexate as the traditional first-line DMARD.

Clinical Pearls
Rapid withdrawal of systemic corticosteroids can trigger life-threatening erythrodermic or pustular psoriasis flares; systemic steroids should be avoided in psoriasis whenever possible. Nail pitting (more than 20 pits) is highly specific for psoriasis and helps distinguish it from onychomycosis; always examine nails. Screen every psoriasis patient for PsA at every visit using validated tools such as the PEST questionnaire, because early treatment prevents joint destruction. IL-23 inhibitors offer the advantage of infrequent dosing (every 8 to 12 weeks) and potential for treatment-free remission, emerging as preferred first-line biologics. Bimekizumab (dual IL-17A/F blockade) achieves the highest complete clearance rates but has approximately a 15 percent rate of oral candidiasis. Cardiovascular risk reduction with biologic therapy is supported by emerging evidence; treat psoriasis as an independent cardiovascular risk factor.
