Opioid Tapering and Deprescribing
Introduction
Opioid tapering refers to the planned, gradual reduction of opioid dose over time, while deprescribing encompasses the broader clinical process of systematically discontinuing medications that are no longer indicated or whose harms outweigh benefits. With growing recognition that long-term opioid therapy for chronic non-cancer pain carries significant risks — including overdose, opioid use disorder, and hormonal disruption — evidence-based tapering has become a core competency for pain medicine practitioners. Critically, the process must be patient-centered, individualized, and supported by multimodal pain management strategies.
Indications for Opioid Tapering
Several situations warrant consideration of an opioid taper. Lack of meaningful functional improvement despite an adequate opioid trial (typically 3-6 months) is a primary indication. Evidence of opioid-related harm — respiratory depression events, recurrent falls, hormonal dysfunction, or persistent sedation — provides clear justification. Signs of opioid use disorder may warrant transition to medication-assisted treatment rather than a simple taper. Opioid-induced hyperalgesia with worsening diffuse pain despite dose escalation is another indication, as is a patient's own request for voluntary dose reduction.
High-dose therapy (above 90 MME per day) without clear functional benefit, concomitant benzodiazepine use or other sedating medications increasing overdose risk, and aberrant drug-related behaviors including early refill requests, lost prescriptions, or positive urine drug screens for unauthorized substances all support the decision to taper.
HHS Guidelines on Opioid Tapering (2019/2022 Updates)
The HHS Guide for Clinicians on the Appropriate Dosage Reduction or Discontinuation of Long-Term Opioid Analgesics emphasizes that tapering should not be abrupt or undertaken without patient agreement when possible. Involuntary rapid tapers and abrupt discontinuation are associated with serious harm including withdrawal crises, uncontrolled pain, psychological distress, and suicidality. Tapers should be individualized based on the patient's opioid dose, duration of use, and psychosocial circumstances. Patients on long-term opioid therapy should not be abandoned — continuity of care during and after the taper is mandatory.
Evidence-Based Tapering Protocols
Slow Taper (Preferred for Most Patients)
The slow taper reduces the dose by 5-10% of the original dose every 2-4 weeks. This rate allows physiologic adaptation and minimizes withdrawal symptoms. For patients on long-term, high-dose therapy, the total taper duration may extend over months to years. Reassessment at each dose reduction for pain, function, mood, and withdrawal symptoms is essential.
Moderate Taper
A moderate taper reduces the dose by 10-20% every 1-2 weeks. This pace is appropriate for patients with clear opioid-related harm requiring more urgent dose reduction. Closer monitoring and more robust multimodal support are needed.

Rapid Taper (Use With Caution)
A rapid taper reduces the dose by 20-50% per week or faster and is reserved for safety emergencies such as severe respiratory depression or acute overdose risk. It should be performed in a monitored setting with access to withdrawal management. Both the CDC and HHS explicitly caution against rapid involuntary tapers in most clinical scenarios.

Practical Dosing Steps
For patients on extended-release formulations, the extended-release component should be decreased first while maintaining a small immediate-release rescue dose. When approaching the lowest available dose, switching to an immediate-release formulation enables finer dose adjustments. The final discontinuation step — from low dose to zero — is often the most challenging, and patients should be allowed extended time at the lowest dose before complete cessation.

Shared Decision Making
Tapering should be framed as a collaborative process rather than a punitive action. Motivational interviewing techniques are valuable: expressing empathy, developing discrepancy between current behavior and goals, rolling with resistance, and supporting self-efficacy. The goals of the taper — improved function, reduced side effects, decreased long-term risk — should be discussed openly. The patient's fear and anxiety about pain returning must be acknowledged and their pain experience validated.
Setting Expectations
Patients should be informed that pain may temporarily increase during the taper but often stabilizes or improves over weeks. The phenomenon of opioid-induced hyperalgesia and how dose reduction may actually reduce pain should be explained. Written taper schedules with clear dose reduction steps and contact information for concerns are helpful. Regular follow-up appointments every 2-4 weeks during active taper provide structure and reassurance.

Managing Withdrawal Symptoms
Common Withdrawal Symptoms
Withdrawal symptoms fall into several categories. Autonomic symptoms include diaphoresis, lacrimation, rhinorrhea, piloerection, and mydriasis. Gastrointestinal symptoms include nausea, vomiting, diarrhea, and abdominal cramping. Musculoskeletal symptoms include myalgias, arthralgias, and restless legs. Neuropsychiatric symptoms include anxiety, insomnia, irritability, and dysphoria.
Pharmacologic Management of Withdrawal
Several medications can manage specific withdrawal symptoms. Clonidine (0.1-0.3 mg three times daily) reduces autonomic symptoms through alpha-2 adrenergic agonism. Loperamide controls diarrhea. Ondansetron or promethazine manages nausea. Trazodone or hydroxyzine addresses insomnia and anxiety. NSAIDs or acetaminophen treats myalgias and arthralgias. Gabapentin may reduce anxiety, insomnia, and pain during the taper.
Non-Pharmacologic Withdrawal Support
Structured exercise programs and physical therapy provide both physiological and psychological benefits during tapering. Cognitive behavioral therapy (CBT) for pain and anxiety management, mindfulness-based stress reduction, and peer support groups all contribute to successful outcomes. Patient education materials reinforce the rationale and help patients navigate difficult periods.
Multimodal Pain Management During Taper
Successful tapering requires concurrent optimization of non-opioid pain management. Non-opioid pharmacotherapy should be initiated or optimized, including acetaminophen, NSAIDs, duloxetine, gabapentinoids, and topical agents. Referral for interventional pain procedures (injections, nerve blocks, neuromodulation) should be made when indicated. Coordination with behavioral health for pain psychology, CBT, and acceptance and commitment therapy is valuable. Physical rehabilitation programs improve function and self-efficacy. Complementary therapies including acupuncture, massage, and yoga can serve as additional supports.

When to Pause or Reverse a Taper
Several situations warrant pausing or reversing a taper. Significant functional decline not explained by other factors is one indication. Severe psychiatric destabilization including suicidal ideation requires immediate reassessment. Acute pain exacerbation from new injury or disease progression may necessitate a pause. When patient distress exceeds the capacity of current support measures, slowing the taper is appropriate. Pausing — maintaining the current dose for an extended period — is always preferable to abrupt discontinuation.
Clinical Pearls
Rapid involuntary tapers should never be pursued — the HHS guidelines explicitly warn that forced rapid tapers cause patient harm including suicidality and transition to illicit substances. The rate of taper should be driven by patient response, not by arbitrary timelines or institutional mandates. A multimodal pain management plan should always be in place before initiating an opioid taper. The clinical rationale for tapering, the patient's informed consent, and the tapering plan should all be documented in detail. Patients who develop compulsive use patterns during the taper may need transition to buprenorphine-based treatment rather than continued tapering.
