Lecture 14: Health Disparities: Race, Class, Gender, and Geography

Lecture Content

I. Defining Health Disparities

A health disparity is a difference in health outcomes that is closely linked to social, economic, or environmental disadvantage. Not all health differences are disparities -- disparities are systematic, avoidable, and unjust. Health equity, by contrast, is the absence of unfair, avoidable differences in health among social groups. Equity is not the same as equality: equality gives everyone the same resources, while equity gives everyone what they need to achieve comparable outcomes.

Understanding health disparities requires looking beyond individual risk factors. Smoking, diet, and exercise matter, but they are themselves shaped by social conditions. A person's health is determined more by their zip code than their genetic code. The social determinants of health, as conceptualized by the WHO framework, are the conditions in which people are born, grow, live, work, and age -- the "causes of the causes."

II. Social Determinants of Health

Income and socioeconomic status are the single strongest predictors of health across nearly all outcomes and populations. The income-health gradient is remarkably consistent: at every level of income, those with more money are healthier than those with less. The mechanisms include access to nutritious food, safe housing, education, and healthcare; reduced exposure to environmental hazards; and reduced chronic stress.

Education is closely linked to health. Higher education is associated with better health literacy, healthier behaviors, higher income, and longer life. Education shapes employment opportunities, social networks, and access to information.

Employment and working conditions affect health through multiple pathways. Unemployment is associated with poor mental and physical health. Hazardous occupations in construction, agriculture, and mining disproportionately employ marginalized workers. Precarious employment -- gig work without benefits -- limits access to employer-sponsored health insurance.

Housing and neighborhood conditions are powerful determinants of health. Environmental exposures such as pollution, lead, mold, and unsafe water (as seen in Flint, Michigan) directly affect health. Food deserts -- neighborhoods without access to affordable, nutritious food -- contribute to diet-related disease. Neighborhood violence and crime cause chronic stress, PTSD, and restricted physical activity.

Social support and community cohesion are protective factors. Strong social networks buffer against illness, while social isolation is a risk factor comparable to smoking.

Access to healthcare encompasses insurance status, proximity to providers, availability of specialists, transportation, and language access. The United States has the highest healthcare spending in the world but ranks poorly on many health outcomes, in part because of unequal access.

III. Race and Health

Racial health disparities in the United States and Canada are pervasive and persistent. Black Americans have two to three times higher maternal mortality than White Americans, along with higher rates of infant mortality, cardiovascular disease, diabetes, and cancer mortality. Indigenous populations in the US and Canada have lower life expectancy and higher rates of chronic disease, mental health disorders, and substance use disorders. These disparities persist even after controlling for income, education, and insurance.

Race is a social construct, not a biological one. There is more genetic variation within racial groups than between them. Race as a variable in medicine is a proxy for social experiences -- racism, poverty, and stress -- not for biology. Race-based medicine, such as race correction in eGFR or spirometry, has been challenged for encoding racist assumptions and delaying diagnosis in Black patients.

Structural racism is a determinant of health. It operates through the ways in which systems and institutions produce and maintain racial inequity through policies, practices, and norms. Historical examples include slavery, Jim Crow laws, redlining, hospital segregation, exclusion from medical training, and the Tuskegee syphilis study. Contemporary manifestations include residential segregation, mass incarceration, environmental racism (in which toxic waste sites are placed near minority communities), and differential treatment in healthcare settings.

The weathering hypothesis, proposed by Arline Geronimus, offers a biological mechanism linking racism and health. Chronic exposure to racism and socioeconomic adversity causes accelerated biological aging -- increased allostatic load -- in Black Americans. This explains health decline and early onset of chronic disease even among middle-class Black Americans.

IV. Gender and Health

Gender disparities operate on multiple levels. Biological sex differences in hormones, genetics, and physiology influence disease prevalence and treatment response. Gender as a social determinant -- including socialization, roles, expectations, power dynamics, and discrimination -- independently shapes health.

Women have been historically underrepresented in clinical research, excluded from many trials until the 1990s. Gender bias in diagnosis means women's symptoms are more likely to be attributed to psychological causes; heart attacks in women, for instance, are more often misdiagnosed. Maternal mortality in the United States remains persistently high compared to peer nations, with stark racial disparities. Gender-based violence, including intimate partner violence, is a major determinant of women's health worldwide.

LGBTQ+ populations experience higher rates of mental health disorders, substance use, and suicidality, driven by minority stress from discrimination, stigma, and rejection. Barriers to healthcare include provider ignorance, discrimination, and a lack of culturally competent care. Transgender individuals face particular disparities in access to gender-affirming care, mental health, and primary care, along with high rates of violence and homelessness.

V. Geographic Disparities

Rural populations face distinctive health challenges, including provider shortages, longer travel distances, hospital closures, limited specialty care, and lower insurance rates. Rates of chronic disease, accidental injury, and opioid-related deaths are higher in rural areas, and the rural-urban health gap is growing rather than shrinking.

Global disparities are even more stark. Life expectancy ranges from under fifty-five years in some sub-Saharan African countries to over eighty-five years in Japan, driven by poverty, conflict, lack of infrastructure, infectious disease burden, and climate vulnerability. The ethical dimensions of global health, including the obligations of wealthy nations, the brain drain of healthcare workers, and medical colonialism, are explored in Lecture 15.

VI. The Physician's Ethical Obligations

Physicians have ethical obligations to address health disparities at three levels. At the individual level, physicians should recognize and mitigate implicit bias in clinical decision-making, provide culturally humble and patient-centered care, and screen for social determinants such as food insecurity, housing instability, and domestic violence, connecting patients to resources.

At the institutional level, physicians should advocate for a diverse healthcare workforce, equitable hiring, and inclusive institutional culture. They should support quality improvement initiatives that track outcomes by race, gender, and socioeconomic status, and ensure language access and culturally appropriate services.

At the systemic level, physicians should advocate for policies that address root causes, including universal healthcare, living wages, affordable housing, environmental regulation, and anti-discrimination legislation. The AMA, CMA, and other medical organizations increasingly recognize health equity as a professional obligation. Medicine alone cannot solve health disparities, but medicine has a unique moral authority and obligation to speak out -- the physician as citizen.
