I. What Are Health Disparities?
Health disparities (or health inequalities) are systematic, avoidable, and unjust differences in health outcomes and their determinants between groups of people. They are distinct from simple health differences: disparities are specifically linked to social, economic, and environmental disadvantage. Health disparities exist along multiple axes, including socioeconomic status (income, education, occupation), race and ethnicity, gender and sex, sexual orientation and gender identity, geographic location (urban versus rural, regional variation), disability status, and immigration status.
Health disparities are not the result of individual choices alone. They are produced and maintained by social structures and institutions. Health equity is the principle that everyone should have a fair opportunity to attain their full health potential, and no one should be disadvantaged from achieving this potential because of their social position.
II. Socioeconomic Disparities in Health
Social class is one of the strongest and most consistent predictors of health. The social gradient in health shows that health improves at each step up the socioeconomic ladder. This is not simply a matter of poverty versus affluence -- the gradient is continuous across the entire class spectrum. The Whitehall Studies, conducted by Michael Marmot, demonstrated this powerfully: even among British civil servants, none of whom were poor, higher-ranking employees had better health than lower-ranking ones.
Several mechanisms link socioeconomic status to health. Material and structural pathways include access to healthcare, nutrition, safe housing, and clean environments. Behavioral pathways involve health behaviors such as smoking, diet, and exercise, which are shaped by resources and social context. Psychosocial pathways involve chronic stress from economic insecurity, low social status, and lack of control. Chronic activation of the stress response through cortisol damages cardiovascular, immune, and metabolic systems. Cumulative disadvantage describes how the health effects of poverty accumulate over the life course: childhood poverty has lasting effects on adult health even if economic conditions improve later.
III. Racial and Ethnic Health Disparities
Profound disparities exist between racial and ethnic groups in the United States. Black Americans have higher rates of heart disease, stroke, diabetes, cancer, and infant mortality, with a life expectancy approximately 5 years less than white Americans. Hispanic and Latino Americans have higher rates of diabetes and certain cancers, though the "Latino/Hispanic paradox" shows some health outcomes that are better than expected given their socioeconomic status. Native Americans have the highest rates of diabetes, substance abuse, and suicide, and the lowest life expectancy of any racial group. Asian Americans generally have better health outcomes on average but exhibit significant variation within the group.
Understanding why racial health disparities exist requires recognizing that race is a social construct, but racism produces real biological effects. Structural racism operates through residential segregation, unequal school funding, occupational segregation, and environmental racism. Differential access to healthcare manifests in uninsured rates, quality of available providers, and geographic access. Provider bias, both implicit and explicit, affects diagnosis, treatment, and pain management -- studies have shown that Black patients' pain is systematically undertreated.
The weathering hypothesis, developed by Arline Geronimus, proposes that chronic exposure to social and economic adversity accelerates biological aging in Black Americans. Allostatic load describes the cumulative physiological toll of chronic stress on the body, including elevated cortisol, inflammation, and cardiovascular wear and tear. John Henryism, a concept from Sherman James, describes how high-effort coping with chronic stress produces health costs. Historical trauma refers to the accumulated emotional and psychological harm across generations from historical events such as slavery, colonialism, and forced relocation. Emerging evidence from epigenetics suggests that social adversity can produce biological changes that may be transmitted across generations.

IV. Gender, Sexuality, and Health Disparities
Gender disparities in health are significant. Women live longer than men but report more morbidity, including chronic illness, disability, and depression. Men have higher rates of heart disease, lung cancer, liver disease, suicide, and substance abuse. Gender norms influence health behaviors: men are less likely to seek medical care, and "masculine" norms discourage help-seeking. Women's health research has historically been underfunded and male-centric, and gender bias in clinical settings means women's symptoms are more often dismissed or attributed to psychological causes.
LGBTQ+ health disparities include higher rates of depression, anxiety, substance use, and suicidality. The minority stress model, developed by Ilan Meyer, explains how chronic stress from stigma, discrimination, and violence produces adverse health outcomes. Barriers to care include discrimination by providers, lack of culturally competent care, and insurance gaps. Transgender health is marked by limited access to gender-affirming care and higher rates of violence, housing instability, and mental health issues.
V. Geographic and Other Disparities
Rural versus urban health presents distinct patterns. Rural areas have fewer healthcare providers, longer travel distances to care, higher rates of chronic disease, and lower life expectancy. Urban areas offer greater access to care but also concentrated poverty, pollution, and violence. Provider shortages disproportionately affect rural areas designated as healthcare professional shortage areas.
Global health disparities are even more stark. Life expectancy can differ by more than 30 years between the highest- and lowest-income nations. Infectious disease, maternal mortality, and child mortality are concentrated in low-income nations, and access to essential medicines, clean water, and sanitation varies enormously.
Immigrant health presents a distinctive pattern. The "healthy immigrant effect" describes how immigrants often arrive healthier than the native-born population. However, their health deteriorates over time with assimilation, as they adopt unhealthy behaviors and experience discrimination. Undocumented immigrants face extreme barriers to healthcare access.
VI. Frameworks for Understanding Health Disparities
Fundamental cause theory, developed by Bruce Link and Jo Phelan, argues that socioeconomic status is a "fundamental cause" of disease because it shapes access to resources that protect health. Even as specific diseases and risk factors change over time, the association between socioeconomic status and health persists because people with more resources are better able to avoid risks and adopt protective strategies regardless of the specific health threat.
Ecosocial theory, developed by Nancy Krieger, holds that health is a product of the ongoing interplay between biological processes and social conditions. People literally "embody" their social experiences: poverty, racism, and discrimination become biologically embedded. The focus is on understanding how social inequality gets "under the skin."
The life course perspective emphasizes that health is shaped by cumulative exposures across the entire lifespan. Critical periods, particularly prenatal development and early childhood, are especially sensitive to social conditions. Risk accumulates over time, and intergenerational transmission means that parents' social conditions affect their children's health.

VII. Reducing Health Disparities
Individual-level interventions alone are insufficient to address health disparities; structural change is needed. Policy approaches include expanding health insurance coverage and access to care, addressing social determinants such as poverty, housing, education, and food access, enforcing anti-discrimination laws in healthcare, investing in community health centers and safety-net providers, and environmental regulation to reduce disproportionate exposure to hazards.
Healthcare system changes include cultural competency and implicit bias training for providers, diversifying the healthcare workforce, implementing patient-centered and community-based care models, and making health equity an explicit institutional goal.
Community-level approaches include deploying community health workers and peer health educators, empowering communities through organizing for health equity, and addressing the built environment by creating safe neighborhoods, parks, and healthy food access.
For pre-medical students, understanding health disparities is foundational for ethical, effective, and equitable medical practice.
