# Managing Complex Multi-Morbidity

## Year 4: Sub-Internship Medicine

Patients with multiple chronic conditions represent an increasing proportion of hospitalized patients and present unique challenges in clinical management. The interaction between multiple diseases creates complexity that cannot be addressed by simply applying single-disease guidelines to each condition. This seminar provides a framework for prioritizing problems, managing competing treatment goals, optimizing medications, and coordinating care for patients with complex multi-morbidity. The emphasis is on patient-centered care that aligns medical decision-making with individual patient values, goals, and preferences.

## Understanding Multi-Morbidity

Multi-morbidity is defined as the presence of two or more chronic conditions in the same individual, though clinically significant complexity typically involves more conditions and their interactions. The prevalence increases substantially with age, affecting more than half of adults over 65 years. The impact extends beyond the sum of individual conditions, as diseases interact to amplify symptoms, complicate management, and worsen outcomes. Quality of life is often significantly affected, with functional limitation and symptom burden reducing what patients are able to do and enjoy. Understanding the patient's multi-morbidity pattern provides essential context for clinical decision-making.

Common patterns of multi-morbidity reflect shared pathophysiology and risk factors. The cardiovascular cluster includes heart failure, coronary artery disease, chronic kidney disease, and diabetes, with each condition worsening the others. Metabolic syndrome encompasses obesity, diabetes, hypertension, and hyperlipidemia, driven by common underlying mechanisms. Frailty syndrome involves the interconnection of weakness, falls, and cognitive decline. Respiratory clusters link chronic obstructive pulmonary disease with heart failure and obstructive sleep apnea. Cancer superimposed on chronic disease creates particularly complex management challenges.

The challenges of managing multi-morbidity arise from multiple sources. Guideline conflicts occur when recommendations for one condition contradict those for another, such as fluid restriction for heart failure conflicting with medication requirements for another condition. Drug interactions multiply as more medications are added, increasing the risk of adverse effects. Competing priorities force difficult decisions about which conditions to address first and how aggressively to treat each. Treatment burden accumulates with multiple medications, dietary restrictions, and monitoring requirements, potentially overwhelming patients. Prognostic uncertainty complicates planning when the trajectory of multiple interacting conditions is unpredictable.

Geriatric syndromes represent common manifestations of multi-morbidity that transcend individual diagnoses. Frailty involves weight loss, weakness, and slowness that predict adverse outcomes independent of specific diagnoses. Falls result from the interaction of multiple risk factors including medications, sensory impairment, and environmental hazards. Delirium represents acute confusion that may be precipitated by infections, medications, or metabolic disturbances in vulnerable patients. Incontinence affects dignity and independence. Functional decline in activities of daily living threatens the ability to live independently. These syndromes require attention alongside the underlying chronic conditions.

## Problem Prioritization

A prioritization framework helps organize the approach to patients with many active problems. Life-threatening conditions demand immediate attention regardless of other considerations. Symptom-causing problems significantly affecting quality of life warrant high priority because they matter most to patients. Potentially reversible conditions present opportunities for meaningful improvement. Preventive care is context-dependent, with value decreasing as life expectancy shortens. Stable, well-optimized chronic conditions can receive lower priority during acute hospitalizations.

Patient-centered priorities recognize that what matters to the patient should drive medical decision-making. Directly asking patients what matters most to them reveals priorities that may differ from medical assumptions. Functional preservation allowing continued independence often ranks higher than disease control for patients. Addressing distressing symptoms may be more important than laboratory optimization. Treatment goals should align with patient values regarding quality versus quantity of life. Treatment burden should be explicitly considered, as even beneficial interventions may not be worthwhile if they overwhelm the patient.

Prognosis-based planning adjusts the intensity and goals of care to expected survival. Patients with years of expected survival may benefit from full preventive care and aggressive disease management. Patients with prognosis measured in months to years benefit most from focus on function and symptom control. Patients with prognosis of weeks to months should have comfort and quality of life as primary goals. Patients approaching the end of life benefit from palliative focus. When prognosis is uncertain, a flexible approach with frequent reassessment is appropriate.

Deprescribing considerations identify medications that may be appropriate to stop. Limited life expectancy reduces the benefit of preventive medications like statins and bisphosphonates, which require years to provide meaningful benefit. Medications causing harm through side effects should be reconsidered. Medications continued without clear indication, often inherited from prior providers, should be questioned. Changed goals of care shifting from aggressive to comfort-focused management warrant medication review. Drug interactions may be eliminated by stopping one of the interacting medications.

## Polypharmacy Management

Polypharmacy is typically defined as the use of five or more medications, with hyperpolypharmacy referring to ten or more. However, the appropriateness of medications matters more than the count. Appropriate polypharmacy means all medications are indicated, effective, and necessary. Inappropriate polypharmacy involves unnecessary, duplicative, or harmful medications. Medication burden encompasses the physical, cognitive, and financial impact of managing multiple medications. The goal is not necessarily fewer medications but rather optimization of the regimen to maximize benefit and minimize harm.

Systematic medication review should be conducted for all patients with polypharmacy. Creating a complete list includes all prescription medications, over-the-counter products, and supplements. For each medication, the indication should be identified and documented. Efficacy assessment determines whether the medication is achieving its intended effect. Safety review considers side effects, interactions, and monitoring requirements. Necessity evaluation questions whether the medication is still needed or if the indication has resolved. Simplification opportunities include combining doses, eliminating duplications, or using combination products.

High-risk medications warrant particular attention in patients with multi-morbidity. Anticholinergic medications including diphenhydramine and oxybutynin cause confusion, urinary retention, and falls. Sedative-hypnotics including benzodiazepines and zolpidem increase fall risk and cognitive impairment. Opioids carry risks of sedation, falls, constipation, and respiratory depression. Anticoagulants require careful monitoring and have significant bleeding risks. Hypoglycemic agents particularly sulfonylureas and insulin can cause dangerous hypoglycemia in patients with variable intake. NSAIDs worsen kidney function, exacerbate heart failure, and increase gastrointestinal bleeding risk.

The deprescribing process follows a systematic approach. Identification of potentially inappropriate medications uses criteria like the Beers list and clinical judgment. Assessment weighs the risks of continuing versus stopping each medication. Prioritization determines which medications to address first based on potential for harm and patient preference. Planning for discontinuation includes tapering schedules when needed to avoid withdrawal or rebound. Monitoring after discontinuation watches for withdrawal symptoms, rebound of underlying conditions, and overall patient response. Communication with patients, families, and other providers ensures coordinated care.

## Drug-Drug Interactions

Common drug-drug interactions should be recognized and managed. Warfarin combined with NSAIDs dramatically increases bleeding risk. ACE inhibitors combined with potassium-sparing diuretics risk hyperkalemia. Multiple QT-prolonging drugs together increase arrhythmia risk. Combinations of serotonergic medications risk serotonin syndrome. CYP450 inhibitors and inducers alter the metabolism of many medications. These interactions require either avoiding the combination, adjusting doses, or implementing enhanced monitoring.

Drug-disease interactions occur when a medication worsens a comorbid condition. NSAIDs worsen chronic kidney disease, exacerbate heart failure, and increase gastrointestinal bleeding risk. Beta-blockers may worsen asthma or severe COPD. Metformin requires dose adjustment or avoidance based on renal function. Anticholinergic medications worsen dementia and can precipitate acute urinary retention in patients with benign prostatic hyperplasia or acute angle-closure glaucoma. Alpha-blockers may worsen orthostatic hypotension. Each medication should be reviewed for potential interactions with the patient's other conditions.

The Beers Criteria provide guidance on potentially inappropriate medications in older adults. Some medications should be avoided entirely in the elderly, including benzodiazepines and first-generation antihistamines. Other medications should be used with caution, with careful monitoring for adverse effects. Drug-disease interactions specify medications to avoid with particular conditions, such as anticholinergics in patients with dementia. Drug-drug interactions to avoid include the combination of opioids and benzodiazepines. The Beers Criteria should inform prescribing but do not replace clinical judgment about individual patients.

STOPP/START criteria complement the Beers Criteria with guidance on both stopping and starting medications. STOPP criteria identify medications that should be stopped, such as proton pump inhibitors continued beyond eight weeks without clear indication, loop diuretics used solely for ankle edema without heart failure, and anticholinergics in patients with dementia. START criteria identify medications that should be started, such as ACE inhibitors in patients with heart failure, statins after myocardial infarction, and vitamin D supplementation in patients at risk for falls. These criteria provide a more balanced approach to medication optimization.

## Goals of Care Discussions

Initiating goals of care conversations requires preparation and sensitivity. Asking permission opens the discussion respectfully, using language such as "Can we discuss what's most important to you as we plan your care?" Understanding the patient's perspective before offering medical recommendations ensures patient-centered communication. Exploring values, fears, and hopes provides context for medical decision-making. Explaining the medical situation honestly allows patients to make informed decisions. Making recommendations based on the patient's expressed goals closes the loop between understanding and action.

The goals framework categorizes different types of treatment objectives. Curative goals aim to eliminate disease entirely. Disease-modifying goals aim to control disease and slow progression. Functional goals aim to maintain independence and ability to do meaningful activities. Symptomatic goals aim to provide comfort and quality of life. Life-prolonging goals aim to extend survival. Most patients have multiple goals that may shift over time, and clarifying the relative priority of different goals guides treatment decisions.

Code status discussions should occur in the context of broader goals of care conversations. The discussion should be prompted by serious illness, not presented as a routine administrative task. CPR outcomes should be presented realistically, with survival to hospital discharge varying significantly by patient population. Patient values regarding quality of life and acceptable states of health should be elicited. The decision should be clearly documented in the medical record. Code status should be revisited as the patient's condition and goals evolve.

Documentation of goals of care conversations creates a record for future reference and team communication. A goals of care note summarizes the discussion content and conclusions. Code status orders document decisions as actionable medical orders. Patient understanding is recorded, including what the patient expressed about their goals and preferences. Decision-makers including any surrogates are documented. Follow-up plans indicate when the conversation will be revisited.

## Care Coordination

Specialist involvement in complex patients creates coordination challenges. When multiple specialists are involved, a primary coordinator is essential to synthesize input and prevent fragmentation. Conflicting advice from different specialists requires reconciliation to create a coherent plan. Communication gaps between specialists may result in missed information or duplicated efforts. The primary team must synthesize specialist input into an integrated plan that makes sense for the whole patient. Duplication of testing and interventions should be avoided through communication and coordination.

Care conferences bring stakeholders together to align understanding and planning. The purpose is to ensure the patient, family, and all team members share the same understanding of the situation and goals. Participants include the patient if able, family or other decision-makers, and relevant team members. The agenda covers current status, goals of care, and specific planning decisions. The outcome is a unified plan that all stakeholders understand and support. Documentation in shared notes ensures continuity and communication.

Transitions of care represent high-risk periods for patients with multi-morbidity. Admission medication reconciliation ensures an accurate understanding of the pre-hospitalization regimen. Discharge requires clear instructions with specific guidance for each condition and medication. Facility transfers require complete information transfer to the receiving team. Specialist referrals should include a clear question and relevant background. Primary care provider follow-up requires timely communication of hospitalization details and ongoing needs.

Social determinants of health significantly impact patients with multi-morbidity. Housing must be safe and appropriate for the patient's functional status. Food security affects the ability to follow dietary recommendations and maintain nutrition. Transportation access affects ability to attend appointments and obtain medications. Social support including caregiver availability affects the ability to manage complex regimens. Financial resources affect medication affordability and access to care. Assessing and addressing social determinants is essential for effective care planning.

## Functional Assessment

Activities of daily living assessment measures basic self-care abilities. Bathing, dressing, toileting, transferring, continence, and feeding represent the core ADLs. Each is assessed as independent, requiring assistance, or dependent. ADL assessment informs discharge planning and care needs. Decline from baseline during hospitalization may indicate delirium, deconditioning, or disease progression. Functional status often matters more to patients than specific diagnoses or laboratory values.

Instrumental activities of daily living assess higher-level function required for independent community living. Using the telephone, shopping, food preparation, housekeeping, laundry, transportation, medication management, and financial management are assessed. IADLs require more cognitive and physical capability than basic ADLs. IADL impairment may develop before ADL impairment and indicates need for support. Assessment guides discharge planning and identifies needed support services.

Mobility assessment identifies fall risk and rehabilitation needs. The Timed Up and Go test measures the time to rise from a chair, walk three meters, turn, walk back, and sit down. Time greater than 12 seconds indicates increased fall risk. Gait speed less than 0.8 meters per second is associated with frailty. Chair stand testing assesses functional strength. Balance tests identify specific deficits. Physical therapy evaluation provides comprehensive mobility assessment and treatment recommendations.

Cognitive screening identifies impairment that affects capacity and self-management ability. The Mini-Cog is a brief three-minute screen combining three-item recall and clock drawing. The Montreal Cognitive Assessment (MoCA) provides more detailed assessment in approximately 10 minutes. The Mini-Mental State Examination (MMSE) is the traditional screening tool. Clock drawing tests executive function. Informant report from family or caregivers provides valuable collateral information about cognitive changes over time.

## Discharge Planning

Discharge readiness encompasses multiple domains beyond medical stability. Medical readiness means symptoms are controlled and the acute issue is resolved or stable. Functional readiness means the patient can perform required activities of daily living or has appropriate support. Cognitive readiness means the patient understands discharge instructions or has a caregiver who does. Social readiness means a safe environment with appropriate support is available. Services readiness means needed home health, equipment, or other services are arranged. Follow-up readiness means appointments are scheduled and accessible.

Disposition options should match patient needs to available resources. Home without services is appropriate for patients who are independent with stable conditions and adequate support. Home with services including home health nursing, physical therapy, home aides, or equipment is appropriate for patients who need assistance but can live at home. Rehabilitation including acute rehabilitation or subacute SNF rehabilitation is appropriate for patients who need intensive therapy and have potential to improve. Skilled nursing facility placement is appropriate for patients who need ongoing nursing care or supervision. Hospice is appropriate for patients with prognosis of six months or less who choose comfort-focused care.

Medication reconciliation at discharge is critical for patient safety. Comparison of admission medications, current medications, and planned discharge medications identifies all changes. Intentional changes should be clearly documented with reasons. Unintentional discrepancies should be corrected. Explanations to patient and family should cover each change in understandable terms. Communication to primary care provider and pharmacy ensures continuity. Simplification of the regimen when possible improves adherence.

Patient education at discharge prepares patients for self-management. Diagnosis explanation should be clear and tailored to the patient's level of understanding. Medication teaching using teach-back technique verifies comprehension. Warning signs indicating when to seek medical attention should be explicit and specific to the patient's conditions. Follow-up appointments should be scheduled before discharge with dates and times confirmed. Written instructions in the patient's primary language provide a reference for home.

## Palliative Care Integration

Knowing when to consult palliative care helps optimize care for patients with serious illness. High symptom burden with uncontrolled pain or dyspnea benefits from palliative care expertise. Serious illness with uncertain prognosis prompts goals clarification. Complex decisions weighing treatment versus comfort benefit from facilitation. Family distress with caregiver burden indicates need for support. Palliative care should be integrated early in serious illness, not reserved for end of life.

The distinction between palliative care and hospice is important for appropriate utilization. Palliative care is appropriate at any stage of serious illness and can be provided alongside curative treatment. Hospice is appropriate when prognosis is less than six months and the patient elects comfort-focused care without curative intent. Palliative care can be provided in any setting, while hospice is typically provided at home or in a hospice facility. Palliative care is covered by regular insurance, while hospice is covered by the Medicare hospice benefit with specific requirements. Both focus on quality of life, but hospice specifically addresses end-of-life care.

Symptom management expertise is a core component of palliative care. Pain is managed with opioids, adjuvant medications, and non-pharmacologic approaches. Dyspnea responds to opioids, oxygen when indicated, and comfort measures including fans and positioning. Nausea is treated with antiemetics chosen based on the underlying mechanism. Anxiety is addressed with medications, counseling, and support. Delirium management includes treating reversible causes and providing comfort for irreversible terminal delirium.

Family support is an essential component of palliative care. Information needs are addressed through clear, honest communication about diagnosis, prognosis, and treatment options. Emotional support acknowledges the distress that accompanies serious illness. Practical support through social work connects families with resources. Spiritual support through chaplaincy addresses existential and religious concerns. Respite provides breaks for caregivers who may become exhausted.

Diabetes management is complicated by variable intake and hypoglycemia risk with cognitive impairment. Dementia affects capacity and medication adherence. Fall history requires balancing medications that increase fall risk against treatments that require those medications.

A problem-solving approach provides structure for managing complex cases. Complete problem listing ensures nothing is overlooked. Prioritization identifies what is most important to address now versus later. Conflict identification recognizes where treatments for one condition worsen another. Patient goal exploration ensures treatment aligns with what matters to the patient. Negotiation between competing medical recommendations and patient preferences seeks acceptable compromises. Documentation records the reasoning behind complex decisions.

The team approach leverages diverse expertise. The physician provides medical management and overall coordination. Nursing provides daily care, patient education, and assessment of function and symptoms. Pharmacy reviews medications for interactions, appropriate dosing, and deprescribing opportunities. Social work addresses resources, placement, and psychosocial support. Physical and occupational therapy assess function and safety and provide rehabilitation. Case management coordinates services and disposition planning.

Longitudinal thinking recognizes that complex patients require ongoing rather than episodic care. Trajectory anticipation considers the expected course of multiple interacting conditions. ## Summary

Multi-morbidity affects more than half of adults over 65 and creates complexity through the interaction of multiple chronic conditions. Problem prioritization addresses life-threatening and symptom-causing conditions first, with patient-centered goals guiding decisions about preventive and disease-modifying care. Polypharmacy management involves systematic medication review with attention to high-risk medications and deprescribing when appropriate. Drug interactions including drug-drug and drug-disease interactions require careful attention, with tools like Beers Criteria and STOPP/START providing guidance.

Goals of care conversations should explore patient values and preferences before making treatment recommendations. Care coordination synthesizes specialist input and addresses social determinants that affect treatment success. Functional assessment of ADLs, IADLs, mobility, and cognition informs discharge planning and care needs. Discharge planning matches disposition to patient needs and ensures medication reconciliation, patient education, and follow-up scheduling.

Palliative care should be integrated early in serious illness, providing symptom management expertise and family support alongside disease-directed treatment. The team approach leverages the expertise of physicians, nurses, pharmacists, social workers, therapists, and case managers. Through systematic approaches and patient-centered communication, complex multi-morbid patients can receive care that aligns with their values and optimizes their quality of life.

## Key Terms

**Multi-morbidity**: The presence of two or more chronic conditions in the same individual, with clinically significant complexity arising from condition interactions.

**Polypharmacy**: The use of five or more medications, with appropriateness determined by whether each medication is indicated, effective, and necessary.

**Deprescribing**: The systematic process of tapering, stopping, or reducing medications to minimize harm and improve outcomes.

**Beers Criteria**: A list of potentially inappropriate medications in older adults, developed by the American Geriatrics Society.
