## Case 1: Benign Prostatic Hyperplasia

### Case Presentation
A 68-year-old male presents to his primary care physician with a 2-year history of progressive urinary symptoms. He reports urinary frequency (voiding every 2-3 hours during the day), nocturia (3-4 times per night), weak urinary stream, hesitancy, and a sensation of incomplete bladder emptying. He denies hematuria, dysuria, or incontinence. His International Prostate Symptom Score (IPSS) is 22 (severe). Medical history includes hypertension controlled with lisinopril. Physical examination reveals a non-tender, symmetrically enlarged prostate estimated at 50 grams on digital rectal examination, with a rubbery consistency and no nodules. PSA is 3.8 ng/mL (within normal range for his age). Urinalysis is normal. Post-void residual volume is 120 mL. The diagnosis is benign prostatic hyperplasia with moderate-to-severe lower urinary tract symptoms. Treatment is initiated with combination therapy: tamsulosin (alpha-blocker) for rapid symptomatic relief and finasteride (5-alpha reductase inhibitor) for long-term prostate volume reduction. The patient is counseled that finasteride will reduce his PSA by approximately 50%, which must be accounted for in future prostate cancer screening.

## Case 2: Testicular Torsion

### Case Presentation
A 16-year-old male is brought to the emergency department at 3 AM with sudden onset of severe left testicular pain that awoke him from sleep 4 hours ago. He reports nausea and one episode of vomiting. He denies fever, dysuria, urethral discharge, or recent trauma. Physical examination reveals an anxious adolescent in obvious distress. The left hemiscrotum is edematous and erythematous. The left testis is exquisitely tender, high-riding (elevated compared to the right), and has a horizontal lie. The cremasteric reflex is absent on the left (stroking the inner thigh does not cause testicular elevation). The right testis is normal. Given the high clinical suspicion, the urology team is called immediately for emergency surgical exploration while a scrotal ultrasound with Doppler is performed, which confirms absent blood flow to the left testis. At surgery, the left spermatic cord is found to be twisted 540 degrees. Upon detorsion, the testis initially appears dusky but regains pink coloration after warm saline application, indicating viability. Bilateral orchiopexy is performed to prevent recurrence. The patient recovers well with preservation of the left testis.

### Key Learning Points
- Testicular torsion is a urologic emergency; testicular salvage rates are approximately 90% if surgery occurs within 6 hours, dropping to near 0% after 24 hours
- Classic presentation includes sudden onset of severe testicular pain, nausea/vomiting, high-riding testis, horizontal lie, and absent cremasteric reflex
- Imaging should not delay surgical exploration when clinical suspicion is high; a normal ultrasound does not exclude torsion

## Case 3: Prostate Cancer

### Case Presentation
A 62-year-old African American male presents for routine health maintenance. He has no urinary symptoms. Family history is significant for prostate cancer in his father, diagnosed at age 65. Digital rectal examination reveals a firm nodule in the right posterior lobe of the prostate. PSA is 8.2 ng/mL (elevated). Given the abnormal DRE and elevated PSA, the patient is referred for multiparametric MRI of the prostate, which shows a PI-RADS 5 lesion in the right peripheral zone. MRI-fusion targeted biopsy reveals Gleason 4+3=7 (Grade Group 3) adenocarcinoma involving 4 of 12 cores, with 60% tumor involvement in the most affected core. Staging workup with CT abdomen/pelvis and bone scan shows no evidence of metastatic disease. After multidisciplinary discussion and shared decision-making, the patient elects to undergo robot-assisted radical prostatectomy. Surgical pathology confirms pT2c disease (bilateral involvement) with negative surgical margins. Post-operative PSA is undetectable. The patient is counseled about expected side effects including temporary urinary incontinence and erectile dysfunction, with discussion of rehabilitation strategies.
