# Pharmacology in Vascular Surgery: Antiplatelet, Anticoagulant, and Thrombolytic Agents

## Antiplatelet Agents

### Aspirin (Acetylsalicylic Acid)

Aspirin exerts its antiplatelet effect by irreversibly inhibiting cyclooxygenase-1 (COX-1), which blocks the synthesis of thromboxane A2 (TXA2), a potent promoter of platelet aggregation. This inhibition lasts for the lifetime of the platelet, approximately 7 to 10 days. The typical dose ranges from 81 to 325 mg daily, with low-dose aspirin (81 mg) preferred for chronic use due to equivalent efficacy and a lower risk of gastrointestinal bleeding. Aspirin is a cornerstone in managing peripheral artery disease (PAD), as it reduces major adverse cardiovascular events (MACE) across all atherosclerotic diseases. However, side effects include gastrointestinal bleeding and aspirin-exacerbated respiratory disease. In the perioperative setting, aspirin is generally continued through most vascular procedures and is only held for surgeries with a high risk of bleeding in closed spaces, such as intracranial operations.

### Clopidogrel (Plavix)

Clopidogrel is an irreversible antagonist of the P2Y12 ADP receptor on platelets. It is a prodrug that requires hepatic activation via the CYP2C19 enzyme. Like aspirin, its effect lasts for the lifespan of the platelet. The standard dose is 75 mg daily, often preceded by a loading dose of 300 to 600 mg. Genetic polymorphisms in CYP2C19 affect clopidogrel metabolism, with 2 to 14% of the population being poor metabolizers who experience reduced drug efficacy; in such cases, platelet function testing or alternative agents should be considered. The CAPRIE trial demonstrated that clopidogrel is marginally superior to aspirin in the PAD subgroup. It is commonly used as an alternative in aspirin-intolerant patients or in combination with aspirin as dual antiplatelet therapy (DAPT). Perioperatively, clopidogrel should be held 5 to 7 days before major surgery but is typically continued for endovascular procedures.

### Ticagrelor (Brilinta)

Ticagrelor is a reversible P2Y12 receptor antagonist that acts directly without requiring hepatic activation. Its antiplatelet effect is reversible, with offset occurring within 3 to 5 days after discontinuation. The EUCLID trial compared ticagrelor to clopidogrel in PAD patients and found no superiority, so ticagrelor is not widely used as a primary agent in PAD. It is more commonly employed in acute coronary syndrome (ACS) settings.

### Prasugrel (Effient)

Prasugrel is an irreversible P2Y12 antagonist with more consistent activation than clopidogrel. However, it is contraindicated in patients with prior stroke or transient ischemic attack (TIA), those older than 75 years, and individuals weighing less than 60 kg due to increased bleeding risk. Its use in peripheral vascular surgery is limited.

### Vorapaxar (Zontivity)

Vorapaxar is a protease-activated receptor-1 (PAR-1) antagonist that blocks thrombin-mediated platelet activation. The TRA 2P-TIMI 50 trial showed that vorapaxar reduced limb events in PAD patients but was associated with increased bleeding risk. It is contraindicated in patients with prior stroke or TIA and has seen limited clinical adoption.

### Dual Antiplatelet Therapy (DAPT) in PAD

DAPT, typically aspirin combined with clopidogrel, is commonly prescribed for 1 to 3 months following peripheral stenting, after which aspirin alone is continued. The CASPAR trial found that DAPT did not improve prosthetic below-knee bypass patency. The optimal duration of DAPT after peripheral interventions remains a subject of debate.

### Aspirin + Low-Dose Rivaroxaban (Vascular Dose)

The COMPASS trial demonstrated that combining aspirin (100 mg) with low-dose rivaroxaban (2.5 mg twice daily) reduced MACE and major adverse limb events (MALE) compared to aspirin alone in patients with stable coronary artery disease (CAD) or PAD. Although this combination increased major bleeding, it also reduced mortality. The VOYAGER PAD trial further showed that aspirin plus rivaroxaban after lower extremity revascularization reduced acute limb ischemia and major amputation rates. This combination is emerging as a standard of care following revascularization in PAD.

## Anticoagulant Agents

### Unfractionated Heparin (UFH)

Unfractionated heparin enhances the activity of antithrombin III, which inhibits thrombin (factor IIa) and factors Xa, IXa, and XIa. It is administered intravenously as a weight-based bolus (80 units/kg) followed by continuous infusion (18 units/kg/hr). Monitoring is performed using activated partial thromboplastin time (aPTT), targeting 1.5 to 2.5 times the control value, or anti-Xa levels between 0.3 and 0.7 IU/mL. UFH is the standard intraoperative anticoagulant for vascular procedures, typically given as an 80 to 100 units/kg bolus before arterial clamping. Protamine sulfate reverses UFH completely, with 1 mg neutralizing 100 units of heparin administered within the last 2 to 3 hours, up to a maximum of 50 mg. Complications include heparin-induced thrombocytopenia (HIT), bleeding, and osteoporosis with prolonged use. The half-life of UFH is dose-dependent, generally lasting 60 to 90 minutes at typical doses.

### Low-Molecular-Weight Heparin (LMWH)

Examples of LMWH include enoxaparin and dalteparin. LMWH primarily exerts anti-factor Xa activity, with less anti-thrombin (IIa) effect than UFH. It is administered subcutaneously in weight-based doses and has predictable pharmacokinetics, usually obviating the need for routine monitoring. Anti-Xa levels may be checked in special populations such as obese patients, those with renal failure, or pregnant women. Treatment dosing for enoxaparin is 1 mg/kg twice daily, while prophylactic dosing is typically 40 mg daily. Protamine partially reverses LMWH, achieving approximately 60% reversal. Because LMWH is cleared renally, dose reduction or avoidance is necessary in patients with creatinine clearance below 30 mL/min, where UFH is preferred. LMWH can cause HIT, but the incidence is lower than with UFH.

### Warfarin (Coumadin)

Warfarin is a vitamin K antagonist that inhibits the synthesis of vitamin K-dependent clotting factors II, VII, IX, and X, as well as proteins C and S. Its anticoagulant effect is monitored by prothrombin time/international normalized ratio (PT/INR), with a target INR of 2.0 to 3.0 for most indications. Warfarin’s onset of full effect takes 3 to 5 days because factor II has the longest half-life of approximately 60 hours. Protein C has a short half-life of about 8 hours, so early warfarin therapy creates a transient hypercoagulable state, which explains warfarin-induced skin necrosis and limb gangrene, especially in patients with protein C deficiency or HIT. Warfarin has extensive drug interactions due to CYP metabolism and is affected by dietary vitamin K intake. Reversal options include vitamin K (slow onset, 6 to 24 hours), fresh frozen plasma (immediate but volume-limited), and four-factor prothrombin complex concentrate (PCC, Kcentra), which is preferred for urgent reversal at doses of 25 to 50 units/kg. In vascular surgery, warfarin is indicated for prosthetic heart valves, atrial fibrillation, antiphospholipid syndrome, and certain hypercoagulable states.

### Direct Oral Anticoagulants (DOACs)

#### Direct Thrombin Inhibitor

Dabigatran is a direct thrombin (factor IIa) inhibitor administered twice daily. It is primarily cleared renally (80%) and can be reversed with idarucizumab (Praxbind). Dabigatran prolongs aPTT and thrombin time (TT), with TT being the most sensitive assay.

#### Factor Xa Inhibitors

Rivaroxaban is a direct factor Xa inhibitor given once daily (with food for 15 or 20 mg doses) and undergoes both hepatic and renal metabolism. Apixaban, also a direct Xa inhibitor, is administered twice daily and has the least renal clearance (~25%), making it preferred in chronic kidney disease. Edoxaban is another once-daily direct Xa inhibitor. Reversal of rivaroxaban and apixaban can be achieved with andexanet alfa (Andexxa), although it is expensive and has limited availability; four-factor PCC is a practical alternative. Routine monitoring is generally unnecessary, but calibrated anti-Xa levels can be used if needed.

#### DOAC Perioperative Management

DOACs have short half-lives of approximately 12 hours and should be held 2 to 3 days before surgery, with longer interruption for dabigatran in patients with renal impairment. Bridging anticoagulation is usually not required due to their rapid offset. Resumption typically occurs 24 to 48 hours postoperatively, depending on bleeding risk.

### Alternative Anticoagulants (For HIT)

Argatroban is a direct thrombin inhibitor cleared hepatically, administered by intravenous infusion, and monitored with aPTT. It is preferred in patients with renal failure. Bivalirudin (Angiomax) is another direct thrombin inhibitor with a short half-life of 25 minutes, cleared enzymatically via proteolysis, making it suitable for intraoperative use and safe in both renal and hepatic failure.

## Thrombolytic Agents

### Mechanism

Thrombolytic agents convert plasminogen to plasmin, which degrades fibrin and dissolves clots. Alteplase (tPA) is a recombinant tissue plasminogen activator that is fibrin-selective and is the standard agent for catheter-directed thrombolysis (CDT) in acute limb ischemia (ALI) and deep vein thrombosis (DVT). Tenecteplase (TNKase) is a modified tPA with a longer half-life and greater fibrin specificity, allowing for single bolus dosing and increasing use. Reteplase (Retavase) is a deletion mutant of tPA used in ALI CDT. Streptokinase, a bacterial protein that is non-fibrin-selective and allergenic, is largely historical.

### Catheter-Directed Thrombolysis (CDT) for ALI

In CDT, alteplase is infused at 0.5 to 1.0 mg per hour through a multi-sidehole catheter embedded within the thrombus. Fibrinogen levels are checked every 6 to 8 hours, and thrombolysis is held if fibrinogen falls below 150 mg/dL. A check angiogram is performed at 12 to 24 hours to assess clot lysis. Concomitant subtherapeutic heparin (300 to 500 units per hour) is administered through the sheath to prevent catheter thrombosis. The duration of therapy typically ranges from 12 to 48 hours.

### Contraindications to Thrombolysis

Absolute contraindications include active internal bleeding, recent cerebrovascular events within 2 to 3 months, and intracranial pathology. Relative contraindications encompass major surgery within 10 days, gastrointestinal bleeding within 10 days, uncontrolled hypertension, pregnancy, and coagulopathy. The risk of hemorrhagic stroke with thrombolysis is approximately 1 to 2%.

### Pharmacomechanical Thrombectomy

Pharmacomechanical thrombectomy combines mechanical thrombus disruption or aspiration with reduced-dose thrombolysis. Devices such as AngioJet (rheolytic), Indigo (aspiration), and EKOS (ultrasound-assisted) are used. This approach shortens treatment time and may reduce bleeding compared to CDT alone.

## Key Clinical Pearls

Aspirin is essential for all patients with PAD unless contraindicated, as it reduces major adverse cardiovascular events by approximately 25%. The COMPASS trial has established that combining rivaroxaban 2.5 mg twice daily with aspirin is an emerging standard for PAD patients at high cardiovascular risk, but bleeding risk must be carefully assessed before prescribing. Clopidogrel resistance due to CYP2C19 polymorphisms is clinically significant and should be considered in patients who experience thrombotic events while on clopidogrel. Warfarin-induced skin necrosis results from transient depletion of protein C, so heparin overlap is mandatory when initiating warfarin therapy. For patients with HIT, argatroban is preferred in renal failure, while bivalirudin is favored for intraoperative use. During catheter-directed thrombolysis, fibrinogen levels should be monitored every 6 to 8 hours, and thrombolytic therapy should be held if fibrinogen drops below 150 mg/dL. Four-factor PCC (Kcentra) is the practical agent for urgent reversal of warfarin and DOACs. Protamine sulfate completely reverses unfractionated heparin but only partially reverses LMWH, achieving about 60% neutralization.
