# Health Disparities and Equity in Urologic Care

## Introduction

Disparities in the incidence, access to treatment, and outcomes of urologic diseases are significant and exist across various racial, ethnic, socioeconomic, geographic, and gender groups. These disparities cannot be explained solely by biological differences but instead reflect a complex interaction of social determinants of health, structural racism, implicit bias, and inequities within healthcare delivery systems. For urologists, understanding these disparities is crucial not only to provide equitable care but also to design inclusive research and advocate for systemic changes. This lecture explores the evidence of disparities across major urologic conditions and introduces frameworks aimed at advancing health equity.

## Social Determinants of Health in Urology

Social determinants of health (SDOH) refer to the conditions in which individuals are born, grow, live, work, and age, all of which influence health outcomes. Several key domains particularly affect urologic care. Economic stability plays a critical role; uninsured and underinsured patients often present with more advanced disease and receive care that is less consistent with established guidelines. Education impacts health literacy, which in turn affects patients’ ability to provide informed consent, adhere to treatments, and participate in screening programs. The neighborhood and built environment influence geographic proximity to urologic specialists, surgical centers, and screening programs, thereby affecting access. Healthcare access itself is shaped by insurance status, availability of subspecialty care, and transportation barriers. Finally, social and community context, including cultural beliefs about cancer screening, masculinity, and health-seeking behavior, also significantly influences urologic health outcomes.

## Disparities in Prostate Cancer

### Incidence and Mortality

Black men have the highest incidence of prostate cancer worldwide, with rates approximately 73% higher than those of White men in the United States. Mortality rates among Black men are roughly 2.0 to 2.4 times higher than those in White men. This disparity in mortality has persisted for decades despite overall improvements in prostate cancer outcomes. Contributing factors include later stages at diagnosis, differences in tumor biology such as higher-grade tumors and earlier age of onset, and inequitable access to care.

### Screening Disparities

Black men are underrepresented in major prostate cancer screening trials such as PLCO and ERSPC. Recognizing their higher risk, the American Urological Association (AUA) and National Comprehensive Cancer Network (NCCN) guidelines recommend initiating shared decision-making about screening at ages 40 to 45 for Black men, compared to 50 to 55 for average-risk men. Despite these recommendations, some Black communities have lower rates of prostate-specific antigen (PSA) screening due to mistrust of the medical system, lack of provider recommendation, and access barriers. Historical medical injustices, such as the Tuskegee Syphilis Study, contribute to ongoing mistrust of healthcare institutions.

### Treatment Disparities

Black men are less likely to receive radical prostatectomy and more likely to undergo watchful waiting or receive no treatment, even after adjusting for age, stage, and comorbidities. Access to robotic surgery is lower in safety-net hospitals, which disproportionately serve minority populations. Utilization of active surveillance also differs by race, raising concerns about differential follow-up and adherence. Additionally, Black and Hispanic patients have lower participation rates in clinical trials, limiting the generalizability of research findings.

### Biological Versus Social Factors

There is some evidence suggesting biological differences, such as a higher prevalence of certain genomic alterations in Black men—for example, the TMPRSS2-ERG fusion is less common, while other high-risk variants may be more prevalent. However, studies conducted within equal-access healthcare systems like the Veterans Affairs (VA) system and military healthcare show a significant reduction or elimination of survival disparities, underscoring the predominant role of social determinants. These findings support the conclusion that equal treatment yields equal outcomes in most analyses from equal-access healthcare environments.

## Disparities in Bladder Cancer

While the overall incidence of bladder cancer is higher in White men compared to other racial groups, Black patients tend to present at more advanced stages and experience worse stage-for-stage survival. Delayed diagnosis is a significant factor, as Black patients often wait longer from symptom onset to evaluation. Furthermore, Black patients are less likely to receive radical cystectomy, neoadjuvant chemotherapy, and continent urinary diversion. Disparities in clinical trial enrollment also limit the applicability of treatment data to these populations.

## Disparities in Kidney Cancer

Incidence rates of kidney cancer are elevated among Black, Hispanic, and American Indian/Alaska Native populations. Although Black patients have a higher incidence, they historically demonstrate comparable or even better cancer-specific survival, which some studies attribute to more indolent tumor biology. However, disparities exist in surgical approaches; Black patients are less likely to receive partial nephrectomy, a nephron-sparing procedure, compared to White patients, even when tumors are small. Additionally, the burden of chronic kidney disease (CKD) is disproportionately higher in Black patients after radical nephrectomy, partly due to a higher baseline prevalence of CKD.

## Disparities in Access to Urologic Care

### Geographic Disparities

Urologists tend to be concentrated in urban and suburban areas, creating significant access barriers for rural patients. Greater travel distance to a urologist correlates with delayed care, lower screening rates, and worse outcomes. Although telehealth has partially mitigated geographic barriers, it requires reliable internet access, which is limited in some underserved areas, perpetuating a digital divide.

### Insurance and Socioeconomic Disparities

Uninsured patients are less likely to receive guideline-concordant cancer treatment. The expansion of Medicaid under the Affordable Care Act (ACA) has been associated with earlier-stage cancer diagnoses and improved survival in some studies. However, underinsured patients face barriers to accessing expensive medications such as enzalutamide, abiraterone, and checkpoint inhibitors. Financial toxicity disproportionately affects minority and low-income patients, further limiting access to optimal care.

### Gender Disparities

Women experience longer delays in the diagnosis of bladder cancer because symptoms are often misattributed to urinary tract infections or menstrual causes. Consequently, women tend to present with more advanced bladder cancer and have worse outcomes. Female urologists represent less than 10% of the workforce, and increasing workforce diversity may positively impact patient experience and access.

## Implicit Bias and Provider Factors

Implicit bias refers to unconscious attitudes or stereotypes that influence understanding, actions, and decisions. Studies using the Implicit Association Test (IAT) demonstrate that healthcare providers harbor implicit biases related to race, ethnicity, age, weight, and gender. Such biases can affect clinical decision-making, including pain management, screening recommendations, treatment choices, and clinical trial enrollment. Strategies to mitigate implicit bias include implicit bias training, the use of structured clinical decision tools, audit and feedback of practice patterns, and fostering diverse care teams.

## Frameworks for Advancing Health Equity

### Individual Level

At the individual level, cultural competency and cultural humility training are essential. Awareness of implicit bias and strategies to mitigate it should be incorporated into clinical practice. The use of patient navigators and community health workers can support underserved populations by facilitating access and adherence. Shared decision-making should be conducted with culturally appropriate communication and language-concordant care to ensure patients are fully informed and engaged.

### Institutional Level

Institutions can advance equity by recruiting a diverse workforce through pipeline programs, holistic admissions, and mentorship for underrepresented trainees. Tracking quality metrics by race, ethnicity, language, and insurance status helps identify and address disparities. Equitable clinical trial enrollment can be promoted through community engagement, removal of exclusionary criteria, and establishing satellite trial sites in underserved areas.

### Systems Level

At the systems level, policy advocacy is critical, including efforts to expand insurance coverage, Medicaid expansion, and telehealth parity legislation. Community-based screening programs targeting high-risk and underserved populations can improve early detection. Research funding should be directed toward understanding and eliminating disparities. Additionally, standardized collection of race, ethnicity, language, and social determinants of health data in clinical registries and electronic health records is necessary to monitor and address inequities effectively.

## Key Clinical Pearls

Black men experience the highest incidence and mortality rates of prostate cancer globally, and screening discussions should begin earlier, at ages 40 to 45, according to AUA and NCCN guidelines. Evidence from equal-access healthcare systems demonstrates markedly reduced racial disparities, emphasizing the predominant influence of social determinants over biological factors. Women presenting with hematuria are more likely to face diagnostic delays for bladder cancer due to symptom misattribution. Implicit bias affects clinical decision-making, but awareness and the use of structured decision tools can mitigate its impact. Tracking quality metrics by race, ethnicity, and insurance status is essential for identifying and addressing institutional disparities. Ultimately, eliminating health disparities requires coordinated action at individual, institutional, and systems levels simultaneously.
