# Contrast Media: Selection, Reactions, and Nephropathy Prevention

## Introduction

Iodinated contrast media are indispensable in interventional radiology, enabling visualization of vascular anatomy, organ perfusion, and pathology. However, contrast agents carry risks including allergic-like reactions, contrast-induced nephropathy (CIN), and thyroid dysfunction. Selecting the appropriate agent and implementing preventive strategies are core competencies for the interventional radiologist.

## Classification of Iodinated Contrast Media

### Chemical Properties

**Ionic vs. non-ionic**: non-ionic agents have lower osmolality and fewer adverse reactions. **Monomers vs. dimers**: dimeric agents have higher iodine content per molecule. **Osmolality categories**: **High-osmolar contrast media (HOCM)**: 1,500-2,000 mOsm/kg (e.g., diatrizoate); rarely used today. **Low-osmolar contrast media (LOCM)**: 600-800 mOsm/kg (e.g., iohexol, iopamidol, ioversol). **Iso-osmolar contrast media (IOCM)**: approximately 290 mOsm/kg (e.g., iodixanol).

**Iohexol (Omnipaque)**: widely available LOCM; standard for most angiographic procedures. **Iopamidol (Isovue)**: LOCM with favorable safety profile. **Iodixanol (Visipaque)**: IOCM; preferred in patients at high risk for CIN. **CO2**: gaseous contrast alternative for patients with severe contrast allergy or renal insufficiency.

## Contrast Reactions

### Classification by Severity

**Mild**: urticaria, pruritus, nausea, flushing, headache; self-limited; no treatment required. **Moderate**: diffuse urticaria, bronchospasm, facial edema, mild hypotension; requires pharmacologic intervention. **Severe**: laryngeal edema, severe bronchospasm, anaphylaxis, cardiovascular collapse; life-threatening emergency.

### Pathophysiology

Most reactions are **anaphylactoid** (non-IgE-mediated), involving direct mast cell degranulation and complement activation. True IgE-mediated anaphylaxis is rare but possible. Risk factors: prior contrast reaction (recurrence rate 20-60%), asthma, atopy, cardiac disease.

### Management

**Mild**: observation; antihistamines (diphenhydramine 25-50 mg IV) if symptomatic. **Moderate bronchospasm**: inhaled beta-agonist (albuterol); epinephrine if unresponsive. **Severe/anaphylaxis**: **epinephrine 0.3 mg IM** (1:1,000) is the first-line treatment; repeat every 5-15 minutes as needed. IV fluids for hypotension; vasopressors if refractory. All IR suites must have a fully stocked **emergency crash cart** readily accessible.

### Premedication Protocols

For patients with prior contrast reactions: **Prednisone 50 mg PO** at 13, 7, and 1 hour before procedure. **Diphenhydramine 50 mg PO/IV** 1 hour before procedure. Accelerated protocol (urgent cases): **methylprednisolone 40 mg IV** at 4 hours and 1 hour before, plus diphenhydramine. Premedication reduces but does not eliminate the risk of breakthrough reactions.

## Contrast-Induced Nephropathy

### Definition and Epidemiology

**CIN** (or post-contrast acute kidney injury): rise in serum creatinine of 0.3 mg/dL or greater or 50% or greater from baseline within 48-72 hours of contrast administration. Incidence: 2-7% in general population; up to 20-30% in high-risk patients. Usually self-limited with creatinine peaking at 3-5 days and returning to baseline within 7-14 days.

### Risk Factors

**eGFR less than 30 mL/min/1.73 m2**: highest risk. **eGFR 30-44 mL/min/1.73 m2**: moderate risk. **Diabetes mellitus** with chronic kidney disease. **Heart failure**, hemodynamic instability, hypovolemia. **Nephrotoxic medications**: NSAIDs, aminoglycosides. High contrast volume (risk increases when volume in mL exceeds 3x eGFR).

### Prevention Strategies

**IV hydration** is the cornerstone: normal saline 1 mL/kg/hr for 6-12 hours pre- and post-procedure. Minimize contrast volume; use diluted contrast when possible. Consider **iso-osmolar contrast** (iodixanol) in high-risk patients. **CO2 angiography** as a contrast alternative for arterial procedures. Hold **metformin** for 48 hours post-contrast; recheck renal function before resuming. N-acetylcysteine and sodium bicarbonate have not demonstrated consistent benefit in large trials.

## Special Considerations

### CO2 Angiography

Safe alternative for aortic, visceral, and lower extremity angiography. Avoid in cerebral and upper extremity circulation due to risk of air trapping. Requires dedicated delivery system to prevent contamination with room air.

### Thyroid Considerations

Iodinated contrast can cause thyrotoxicosis in patients with underlying thyroid disease. Screen for hyperthyroidism history; consider endocrinology consultation if concern exists.

## Key Clinical Pearls

Epinephrine IM is the single most important medication for severe contrast reactions; do not delay administration for other interventions. The ACR Manual on Contrast Media recommends against routine creatinine screening in all patients; target screening to those with identifiable risk factors. IV hydration remains the only consistently proven intervention for CIN prevention. For patients with eGFR less than 30 who require contrast, a risk-benefit discussion with documentation is essential; consider CO2 or gadolinium alternatives.
