# Blood Product Transfusion: Evidence-Based Thresholds

## Introduction

Blood product transfusion is one of the most common interventions in hospital medicine. Over the past two decades, landmark trials have shifted practice from liberal to **restrictive transfusion strategies**. Understanding evidence-based thresholds, indications, and risks is essential for safe and effective transfusion practice.

## Red Blood Cell Transfusion

- Always treat the **patient, not the number**; consider symptoms, hemodynamic status, and ongoing bleeding

### Single-Unit Transfusion Policy

- Transfuse **one unit at a time** and reassess before ordering additional units
- Each unit of pRBCs raises hemoglobin by approximately **1 g/dL** in a 70 kg adult
- Single-unit transfusion reduces unnecessary exposure and conserves blood supply

## Platelet Transfusion

### Indications and Thresholds

- **Contraindicated** in thrombotic thrombocytopenic purpura (TTP) and heparin-induced thrombocytopenia (HIT) unless life-threatening hemorrhage

### Platelet Dosing

- One **apheresis unit** (equivalent to 6 pooled whole-blood-derived units) raises count by 30,000-50,000/mcL
- Check post-transfusion count at **1 hour** and **24 hours** to assess response
- **Platelet refractoriness**: consider HLA-matched platelets if two consecutive poor increments

## Fresh Frozen Plasma (FFP)

- Indicated for **active bleeding with coagulopathy** (INR > 1.5-2.0)
- Dose: **10-15 mL/kg** (typically 3-4 units for an adult)
- **Not indicated** for mild INR elevations without bleeding or prior to low-risk procedures
- Consider **4-factor prothrombin complex concentrate (4F-PCC)** for warfarin reversal as it provides faster correction with less volume
- FFP contains all clotting factors but requires ABO-compatible product

## Cryoprecipitate

- Rich in **fibrinogen**, Factor VIII, Factor XIII, von Willebrand factor, and fibronectin
- Primary indication: **fibrinogen < 100-150 mg/dL** in the setting of active bleeding or DIC
- Standard dose: **10 units** (one pooled dose), expected to raise fibrinogen by 50-70 mg/dL
- Also used in massive transfusion protocols and as adjunct in uremic bleeding

## Massive Transfusion Protocols

- Activated when a patient is expected to require **>10 units pRBCs in 24 hours** or **>4 units in 1 hour**
- Goal ratio of **pRBCs : FFP : platelets = 1:1:1** based on the PROPPR trial
- Early activation reduces mortality by ensuring timely availability of balanced products
- Incorporate **point-of-care coagulation testing** (TEG/ROTEM) for goal-directed therapy when available
- Monitor and treat **hypocalcemia**, hypothermia, and acidosis (the lethal triad)

## Transfusion Reactions

### Acute Reactions

- **Acute hemolytic reaction**: fever, hypotension, flank pain; caused by ABO incompatibility; stop transfusion immediately
- **Febrile non-hemolytic reaction**: most common; fever and chills without hemolysis; use leukoreduced products
- **Allergic reactions**: urticaria (mild) to anaphylaxis (severe, especially in IgA-deficient patients)
- **Transfusion-related acute lung injury (TRALI)**: non-cardiogenic pulmonary edema within 6 hours; supportive care only

### Delayed Reactions

- **Delayed hemolytic reaction**: occurs 3-28 days post-transfusion; extravascular hemolysis
- **Transfusion-associated graft-versus-host disease**: rare, fatal; prevented by irradiation of products
- **Iron overload**: in chronically transfused patients; monitor ferritin and consider chelation

## Special Populations

- **Sickle cell disease**: use phenotypically matched, HbS-negative units; avoid overtransfusion (Hb > 10 g/dL)
- **Jehovah's Witnesses**: respect informed refusal; optimize erythropoiesis with iron, EPO, and cell salvage
- **Neonates and pediatrics**: use irradiated, CMV-negative, volume-reduced products

## Key Clinical Pearls

- A restrictive transfusion strategy (Hb < 7 g/dL) is safe and preferred for most hospitalized adults.
- Transfuse one unit at a time and reassess; avoid reflexive multi-unit orders.
- Platelet transfusion is contraindicated in TTP and HIT unless life-threatening hemorrhage is present.
- Massive transfusion protocols with balanced ratios (1:1:1) improve survival in hemorrhagic shock.
- Always report and document transfusion reactions; stop the transfusion and send required laboratory studies.
