# Dementia: Early Recognition, Workup, and Caregiver Support

## Introduction

Dementia affects approximately 7 million adults in the United States and is projected to nearly double by 2060 as the population ages. Family physicians manage the majority of dementia cases and serve as the longitudinal care coordinators for patients and their families. Early recognition allows for timely planning, treatment of reversible contributors, and support for caregivers.

## Definitions

Major neurocognitive disorder, the term now used interchangeably with dementia, refers to a significant decline in one or more cognitive domains such as memory, executive function, language, visuospatial ability, or social cognition that interferes with independence in daily activities. Mild cognitive impairment (MCI) describes a modest decline in cognitive function that does not yet interfere with independence, though it carries an annual conversion rate to dementia of approximately 10 to 15%. Delirium is an acute, fluctuating disturbance in attention and awareness that must always be ruled out before diagnosing dementia, as the two can easily be confused.

## Types of Dementia

Alzheimer disease accounts for 60 to 80% of dementia cases and presents with insidious onset, progressive memory loss, and later decline in executive function and language. Vascular dementia makes up 10 to 20% of cases and typically follows a stepwise pattern of decline, often accompanied by focal neurological findings and evidence of cerebrovascular disease on imaging. Lewy body dementia is characterized by visual hallucinations, parkinsonism, fluctuating cognition, REM sleep behavior disorder, and severe sensitivity to neuroleptic medications. Frontotemporal dementia tends to present earlier, typically between ages 45 and 65, with personality changes, disinhibition, or progressive aphasia. Mixed dementia involves a combination of pathologies, most commonly Alzheimer disease and vascular disease together.

## Early Recognition

### Red Flags in Primary Care

Several clinical signs should raise concern for cognitive decline in primary care. Repetitive questions or stories within the same visit are common early indicators. Difficulty managing medications, finances, or appointments suggests executive dysfunction. Getting lost in familiar places signals visuospatial decline. Personality or behavioral changes noticed by family members, decline in personal hygiene, unexplained weight loss, and increased falls or motor vehicle incidents all warrant further evaluation.

### Screening Tools

The Mini-Cog combines a three-item recall with clock drawing and takes only two to three minutes, making it a highly sensitive and practical screen. The Montreal Cognitive Assessment (MoCA) is a 30-point test evaluating multiple cognitive domains, with a score below 26 suggesting impairment; it is more sensitive than the MMSE for detecting MCI. The MMSE, while widely known, is less sensitive for early impairment and is copyrighted. The Saint Louis University Mental Status (SLUMS) examination offers a free, validated alternative to the MoCA. Regardless of which tool is used, obtaining collateral history from a family member or caregiver is essential, as patients often underestimate their own deficits.

### Annual Cognitive Assessment

The Medicare Annual Wellness Visit includes cognitive assessment as a required element. Brief structured tools such as the Mini-Cog are appropriate for screening during these visits.

## Diagnostic Workup

### Goals of Workup

The diagnostic workup serves three purposes: confirming cognitive impairment and establishing its severity, identifying reversible causes that mimic or worsen dementia, and determining the likely type of dementia to guide management and prognosis.

### Laboratory Studies

Thyroid-stimulating hormone (TSH) should be checked because hypothyroidism can cause cognitive impairment. Vitamin B12 deficiency causes reversible cognitive decline and should be assessed. A complete metabolic panel helps rule out metabolic derangements such as hypercalcemia, hyponatremia, and renal or hepatic failure. A complete blood count identifies anemia that may contribute to cognitive symptoms. Additional studies to consider based on clinical context include folate, RPR or VDRL for neurosyphilis in high-risk populations, HIV testing if risk factors are present, and urinalysis to evaluate for urinary tract infection as a cause of delirium.

### Neuroimaging

Non-contrast MRI of the brain is the preferred imaging modality to assess for vascular disease, structural lesions, and patterns of atrophy. CT of the head is acceptable when MRI is contraindicated. Imaging is recommended at least once during the workup to rule out mass lesions, normal pressure hydrocephalus, and subdural hematoma.

### Advanced Testing (Specialty Referral)

Neuropsychological testing provides a detailed cognitive assessment that characterizes specific deficits and helps differentiate MCI from dementia. Amyloid PET scans and CSF biomarkers measuring amyloid-beta and phospho-tau are useful in atypical or early-onset presentations and are increasingly available, though not yet standard in primary care. FDG-PET and DaTscan may help distinguish Alzheimer disease from frontotemporal or Lewy body dementia.

### Depression Screen

Pseudodementia from depression can closely mimic dementia and should be screened for with the PHQ-9. Treating depression may significantly improve cognition. Depression and dementia frequently coexist, and depression should be treated regardless of the dementia diagnosis.

## Management

### Non-Pharmacologic Interventions

Cognitive stimulation therapy involves group-based activities and has moderate evidence for improving cognition and quality of life. Regular physical exercise, including both aerobic and resistance training, slows functional decline. Structured daily routines reduce confusion and behavioral disturbance. Maintaining social engagement through meaningful activities and connections is beneficial. Environmental modifications to simplify the home, improve safety, and address wandering risk are essential components of the care plan.

### Pharmacotherapy

#### Cholinesterase Inhibitors

Donepezil (5 to 10 mg daily), rivastigmine (available as a patch or oral formulation), and galantamine provide modest symptomatic benefit in mild-to-moderate Alzheimer disease. Common side effects include nausea, diarrhea, bradycardia, and insomnia. The benefit of these medications should be reassessed every six to twelve months.

#### NMDA Receptor Antagonist

Memantine, dosed at 10 mg twice daily or 28 mg extended-release daily, is added in moderate-to-severe Alzheimer disease and is often combined with a cholinesterase inhibitor. It is generally well tolerated, with potential side effects including dizziness, headache, and confusion.

#### Anti-Amyloid Therapies

Lecanemab and donanemab are monoclonal antibodies targeting amyloid-beta that modestly slow cognitive decline in early Alzheimer disease. They require amyloid PET or CSF biomarker confirmation before initiation and are associated with amyloid-related imaging abnormalities (ARIA), necessitating regular MRI monitoring. These therapies are currently administered in specialty settings.

### Managing Behavioral and Psychological Symptoms of Dementia (BPSD)

Non-pharmacologic strategies are first-line and include identifying triggers, redirecting the patient, modifying the environment, providing reassurance, and addressing unmet needs such as pain, constipation, or boredom. Antipsychotics such as risperidone and quetiapine should be used only for severe agitation or psychosis that poses a safety risk, keeping in mind the FDA black box warning for increased mortality in dementia patients; the lowest dose for the shortest duration is the guiding principle. Benzodiazepines should be avoided because they can cause paradoxical agitation, falls, and delirium. Low-dose citalopram may be considered for agitation in Alzheimer disease, based on evidence from the CitAD trial.

## Caregiver Support

### Caregiver Burden

Over 11 million unpaid caregivers provide care for people with dementia in the United States. These caregivers experience higher rates of depression, anxiety, social isolation, and physical health decline. Screening caregivers for depression and burnout at patient visits is an important component of comprehensive dementia care.

### Supporting Caregivers

Education about the disease trajectory, expected progression, and available resources is fundamental. Referral to the Alzheimer's Association (24/7 helpline: 800-272-3900) and local caregiver support groups provides ongoing assistance. Respite care options include adult day programs, in-home respite, and short-term residential respite. Anticipatory guidance should address driving safety, wandering prevention, home safety modifications, and financial and legal planning. Caregiver training in techniques for managing BPSD, communication strategies, and self-care improves outcomes for both the caregiver and the patient.

### Advance Care Planning

Advance care planning should be initiated early while the patient retains capacity for decision-making. Discussions should cover healthcare proxy designation, power of attorney, and advance directives. Goals of care, feeding preferences, and hospitalization preferences should be addressed as dementia progresses, and these conversations should be revisited regularly as the disease advances.

## Key Clinical Pearls

Collateral history from a reliable informant is essential because patients with dementia often lack insight into their deficits. Delirium, depression, and reversible causes should always be ruled out before diagnosing dementia. The Mini-Cog takes under three minutes and is one of the most practical screening tools for busy primary care settings. Driving assessment should be addressed early, as many states require physician reporting when dementia impairs driving safety. Caregiver health is patient health; supporting the caregiver directly improves patient outcomes and delays institutionalization.
