The WHO classification of neuroendocrine neoplasms establishes a framework that is critical for treatment selection and prognostication. Well-differentiated neuroendocrine tumors (NETs) are low to intermediate grade neoplasms that retain neuroendocrine differentiation and express chromogranin A and synaptophysin. These are subdivided by proliferative rate: G1 tumors have a Ki-67 index below 3% and mitotic rate below 2 per 10 high-power fields; G2 tumors have a Ki-67 of 3-20% and mitotic rate of 2-20 per 10 HPF; and G3 tumors have a Ki-67 exceeding 20% and mitotic rate exceeding 20 per 10 HPF. The G3 well-differentiated NET designation is an important category, as these tumors are histologically well-differentiated despite high proliferative rates, and their treatment differs fundamentally from poorly differentiated neuroendocrine carcinomas.

Poorly differentiated neuroendocrine carcinomas (NECs) are high-grade neoplasms of either small cell or large cell type, with Ki-67 typically exceeding 55%. These aggressive tumors are treated with platinum-based chemotherapy analogous to small cell lung cancer protocols. Mixed neuroendocrine-non-neuroendocrine neoplasms (MiNEN) contain both neuroendocrine and non-neuroendocrine components, each comprising more than 30% of the tumor.

### Epidemiology

The incidence of neuroendocrine tumors has been steadily increasing, now estimated at 6-7 per 100,000 per year, largely attributable to improved detection through modern imaging and endoscopic techniques. The prevalence in the US is approximately 170,000, reflecting the long survival of patients with well-differentiated tumors that drives a high prevalent population. By site, lung NETs account for 30%, small intestine 25%, rectum 15%, pancreas 10%, colon 5%, stomach 5%, and appendix 5%. Five-year survival varies dramatically across the spectrum: greater than 95% for localized G1 tumors to below 20% for metastatic G3/NEC.

### Functional vs Non-Functional

Functional NETs, representing 20-30% of cases, secrete bioactive hormones that produce recognizable clinical syndromes, including serotonin (carcinoid syndrome), insulin, gastrin, glucagon, VIP, and somatostatin. Non-functional tumors, comprising the majority at 70-80%, may secrete chromogranin A, pancreatic polypeptide, or other markers without producing a clinical syndrome and typically present through mass effect or incidental discovery.

## Small Intestinal NETs (formerly "carcinoid tumors")

### Characteristics

Small intestinal NETs are the most common site for functioning NETs and account for 25% of all GI NETs. They typically arise in the ileum (60-80%) and are multicentric in 30% of cases. These tumors produce serotonin (5-HT), tachykinins, prostaglandins, and histamine. They often present at an advanced stage, with liver metastases present in 50% at diagnosis, due to their characteristically insidious growth. A pathognomonic feature is the mesenteric fibrosis and desmoplastic reaction that surrounds the primary tumor and its lymph node metastases, which can cause bowel obstruction, mesenteric ischemia, and ureteral obstruction. Despite their metastatic potential, these tumors tend to be indolent, with median survival of 5-8 years even with metastatic disease.

### Carcinoid Syndrome

#### Pathophysiology

Carcinoid syndrome develops when vasoactive substances, primarily serotonin, reach the systemic circulation in sufficient quantities. In most cases, this requires hepatic metastases, because the liver normally metabolizes serotonin via first-pass effect, preventing it from reaching the systemic circulation. Exceptions include retroperitoneal or ovarian tumors that drain directly into the systemic circulation and bronchial NETs that bypass portal circulation entirely. Serotonin is synthesized from tryptophan via tryptophan hydroxylase and metabolized to 5-HIAA by monoamine oxidase. The diversion of tryptophan toward serotonin synthesis and away from niacin production creates the potential for niacin deficiency (pellagra).

#### Clinical Features (Classic Triad: Flushing + Diarrhea + Wheezing)

Flushing, present in 85% of patients with carcinoid syndrome, is episodic and sudden in onset, affecting the face, neck, and upper trunk. It is characteristically dry, warm, and red to violaceous, lasting seconds to minutes, and is triggered by alcohol, exercise, stress, certain foods, and anesthesia. Importantly, flushing is mediated by tachykinins and kallikrein, not serotonin, which is a common misconception.

Diarrhea, present in 80%, is serotonin-mediated, secretory in nature, watery, and nonbloody. Serotonin stimulates intestinal motility and secretion, and the diarrhea can be debilitating with up to 10-20 episodes daily. Bronchospasm occurs in 15-20%, manifesting as wheezing and dyspnea mediated by serotonin and histamine.

Carcinoid heart disease develops in 20-40% of patients with carcinoid syndrome. Serotonin causes endocardial fibrosis, predominantly affecting the right side of the heart because the lungs metabolize serotonin before it reaches the left heart. The characteristic pathology consists of plaque-like deposits of fibrous tissue on the valvular endocardium, producing tricuspid regurgitation and pulmonic stenosis. Screening with echocardiography at diagnosis and annually is recommended for all patients with carcinoid syndrome. NT-proBNP serves as a screening marker, and 24-hour urinary 5-HIAA exceeding 300 umol is associated with higher risk. Valve replacement with bioprosthetic valves is indicated for symptomatic severe valvular disease.

Carcinoid crisis is a life-threatening emergency involving massive release of vasoactive substances during anesthesia, tumor manipulation, biopsy, or chemoembolization. It manifests as severe hypotension (or hypertension), bronchospasm, flushing, and tachycardia. Prevention with prophylactic octreotide infusion at 50-100 mcg/h IV before and during procedures is essential. Pellagra, resulting from niacin deficiency due to tryptophan diversion toward serotonin synthesis, classically presents with dermatitis, diarrhea, and dementia. Though rare, niacin supplementation should be provided for patients with significant serotonin production.

## Diagnosis

### Biochemical Markers

Chromogranin A (CgA) is a general NET marker elevated in 60-90% of NETs that correlates with tumor burden. False positives are common with PPI use (which must be stopped 2 weeks before testing), chronic atrophic gastritis, renal failure, heart failure, and inflammatory conditions, limiting its specificity. Twenty-four-hour urine 5-HIAA (5-hydroxyindoleacetic acid) is specific for serotonin-producing NETs with sensitivity of 70-90% for carcinoid syndrome. Patients must avoid tryptophan-rich foods before collection, including bananas, avocados, pineapple, walnuts, tomatoes, and eggplant. Neuron-specific enolase (NSE) is elevated in poorly differentiated NEC but is less useful for well-differentiated NETs. Specific hormone levels guide evaluation for functional syndromes: fasting gastrin for gastrinoma, fasting insulin/C-peptide/proinsulin for insulinoma, VIP for VIPoma, and glucagon for glucagonoma.

### Imaging

Triphasic CT demonstrates the characteristically hypervascular hepatic metastases with arterial phase enhancement and may reveal the mesenteric primary with its desmoplastic reaction (calcified, stellate mass). MRI is superior for detecting liver metastases through diffusion-weighted imaging and may identify lesions missed by CT. Somatostatin receptor scintigraphy (OctreoScan) using In-111 pentetreotide detects SSTR-expressing tumors with sensitivity of 70-90% but is being largely replaced by the superior modality.

68Ga-DOTATATE PET/CT binds SSTR2 with high affinity and achieves sensitivity exceeding 95% for well-differentiated NETs, far surpassing OctreoScan and CT. It is now the standard of care for staging, restaging, and assessment of eligibility for peptide receptor radionuclide therapy (PRRT), having received FDA approval in 2016. 18F-FDG PET/CT is useful for high-grade NETs (G3) and NEC, which may lose somatostatin receptor expression. FDG-avid tumors indicate more aggressive biology, and the "flip-flop" pattern (FDG-positive/DOTATATE-negative) identifies tumors with the worst prognosis. Endoscopic ultrasound is the most sensitive modality for detecting pancreatic and duodenal NETs smaller than 2 cm and allows tissue biopsy.

### Pathologic Grading

Ki-67 index and mitotic count are the two most important prognostic factors in neuroendocrine neoplasms. Immunohistochemistry for chromogranin A and synaptophysin (positive in NETs) and Ki-67 (MIB-1 antibody) is essential. The distinction between well-differentiated and poorly differentiated neoplasms is critical because it determines the fundamental treatment approach: somatostatin analogs and targeted therapy for well-differentiated tumors versus platinum-based chemotherapy for poorly differentiated carcinomas.

## Treatment

### Surgery

Curative surgical resection with regional lymph node dissection remains the primary treatment for localized NETs. Debulking or cytoreductive surgery for functional NETs with liver metastases reduces hormone secretion and improves symptoms, and is considered when 70% or more of the tumor burden can be resected. Liver-directed therapies including hepatic artery embolization (HAE), transarterial chemoembolization (TACE), and radioembolization (Y-90 microspheres) exploit the predominantly arterial blood supply of hepatic NET metastases for unresectable hepatic disease. Liver transplantation is considered rarely, for young patients with liver-only metastases from well-differentiated NETs, achieving 5-year survival of approximately 50-70% in highly selected cases. Appendiceal NETs smaller than 1 cm that are G1 without mesoappendicular invasion require appendectomy alone; right hemicolectomy is recommended for tumors exceeding 2 cm or with high-risk features.

### Somatostatin Receptor Ligands (SRLs)

#### Antiproliferative and Symptom Control

Octreotide LAR at 20-30 mg intramuscularly every 4 weeks demonstrated antiproliferative activity in the PROMID trial (midgut NET), with median time to progression of 14.3 months versus 6.0 months with placebo, and is FDA-approved for both functional and non-functional NETs. Lanreotide Autogel at 120 mg subcutaneously every 4 weeks showed PFS that was not reached versus 18 months with placebo in the CLARINET trial (non-functional GEP-NET) and is FDA-approved. Above-label dosing of octreotide LAR at 40-60 mg or lanreotide every 3 weeks may be necessary for refractory symptoms. Short-acting octreotide at 100-500 mcg subcutaneously three times daily is used for breakthrough symptoms and pre-procedure carcinoid crisis prevention (50-100 mcg/h IV infusion).

### Peptide Receptor Radionuclide Therapy (PRRT)

177Lu-DOTATATE (Lutathera), FDA-approved in 2018, delivers targeted beta-radiation to SSTR-positive tumor cells. The NETTER-1 trial demonstrated its efficacy in progressive midgut NETs after octreotide LAR: PFS was 28.4 months with 177Lu-DOTATATE plus octreotide LAR 30 mg versus 8.5 months with octreotide LAR 60 mg alone, with an objective response rate of 18% versus 3%. Dosing consists of 7.4 GBq (200 mCi) IV every 8 weeks for 4 cycles, with amino acid infusion for renal protection (reducing proximal tubular reabsorption of the radiopeptide). Eligibility requires DOTATATE PET-positive tumors with adequate renal and hematologic function. Side effects include nausea (from amino acid infusion), myelosuppression (grade 3-4 in approximately 10%, which can be delayed, requiring CBC monitoring for 2 or more years), renal toxicity (rare with amino acid protection), and myelodysplastic syndrome or AML (below 2%). Alpha-emitting 225Ac-DOTATATE PRRT is investigational but potentially more potent for refractory cases.

### Targeted Therapies

Everolimus, an mTOR inhibitor at 10 mg daily, demonstrated improved PFS in multiple settings: 11.0 versus 4.6 months in the RADIANT-3 trial (pNET) and 11.0 versus 3.9 months in RADIANT-4 (non-functional, non-pancreatic NET). It is FDA-approved for progressive GEP-NET and lung NET. Side effects include stomatitis (60%), hyperglycemia, pneumonitis, immunosuppression, and hyperlipidemia. Sunitinib, a multi-kinase inhibitor at 37.5 mg daily continuously, improved PFS from 5.5 to 11.4 months in pancreatic NET and is FDA-approved for progressive pNET. Side effects include fatigue, diarrhea, hand-foot syndrome, hypertension, and myelosuppression.

### Chemotherapy

For well-differentiated pNETs, temozolomide-based regimens, particularly the combination of temozolomide and capecitabine (CAPTEM), achieve objective response rates of 33-70%. Streptozocin-based regimens were the historical standard but are declining in use. For poorly differentiated NEC, platinum-etoposide (cisplatin or carboplatin plus etoposide) achieves response rates of 40-60% with short duration of response. Second-line options include temozolomide-based regimens, FOLFIRI, and FOLFOX. Well-differentiated G3 NETs occupy an intermediate behavior category that may respond to temozolomide-based regimens or PRRT rather than platinum, making the pathological distinction from NEC critical.

### Symptom Management

Diarrhea management begins with SRLs as first-line. Telotristat ethyl (Xermelo), a tryptophan hydroxylase (TPH) inhibitor that directly reduces serotonin synthesis, is dosed at 250 mg three times daily with an SRL. The TELESTAR trial demonstrated significant reduction in bowel movements, establishing it as a valuable addition for refractory carcinoid diarrhea. Flushing is managed with SRLs and dose escalation of octreotide for breakthrough episodes, along with trigger avoidance. Carcinoid heart disease requires regular echocardiographic screening with surgical valve replacement when indicated and perioperative octreotide for crisis prevention. Niacin supplementation is provided for patients with significant serotonin production to prevent pellagra.

### Insulinoma

Insulinoma is the most common functional pNET, with 90% being benign and solitary, and 5-10% associated with MEN1. Diagnosis requires the 72-hour supervised fast demonstrating Whipple triad (hypoglycemia symptoms, documented glucose below 45 mg/dL, and symptom resolution with glucose administration) along with insulin at or above 3 mU/mL, C-peptide at or above 0.6 ng/mL, proinsulin at or above 5 pmol/L, suppressed beta-hydroxybutyrate, and a negative sulfonylurea/meglitinide screen. Localization employs CT, MRI, EUS (most sensitive), 68Ga-DOTATATE PET, and intraoperative ultrasound. Surgical enucleation or distal pancreatectomy is curative in over 95% of benign cases. Diazoxide (which inhibits insulin release) provides temporization before surgery.

### Gastrinoma / Zollinger-Ellison Syndrome

Eighty percent of gastrinomas are located within the "gastrinoma triangle" defined by the CBD-duodenal junction, pancreatic head, and duodenal wall. Twenty-five percent are associated with MEN1; sporadic tumors are more often malignant. Diagnosis requires fasting gastrin exceeding 10 times the upper limit of normal with gastric pH below 2, confirmed by the secretin stimulation test (2 U/kg IV) showing a paradoxical gastrin rise exceeding 120 pg/mL from baseline. PPI-induced hypergastrinemia must be excluded by stopping PPIs 1 week before testing with H2 blocker coverage. Treatment involves PPIs for acid control (often at high doses), with surgical resection if the tumor is localized and sporadic. Cure is rare in MEN1-associated gastrinoma due to the characteristically multiple, small duodenal tumors.

### Glucagonoma

The glucagonoma syndrome includes the characteristic necrolytic migratory erythema (an erythematous, erosive, migrating skin rash affecting the face, groin, perineum, and extremities), diabetes mellitus, weight loss, anemia, deep vein thrombosis, glossitis, and stomatitis. Fasting glucagon exceeds 500 pg/mL (often above 1000). Most glucagonomas are malignant with liver metastases at presentation. Treatment involves surgery if resectable, SRLs for symptom control, zinc and amino acid supplementation for the skin rash, and anticoagulation for DVT risk.

### VIPoma (Verner-Morrison Syndrome/WDHA)

VIPoma presents with profuse watery diarrhea (exceeding 3 L/day), hypokalemia, and achlorhydria (WDHA). VIP exceeds 200 pg/mL, and other causes of secretory diarrhea must be excluded. Most are pancreatic, often large, and metastatic at diagnosis. Treatment requires aggressive fluid and electrolyte replacement, SRLs (effective for symptom control), and surgery if resectable.

## Key Clinical Pearls

- 68Ga-DOTATATE PET/CT has revolutionized NET imaging with >95% sensitivity for SSTR-positive tumors; it is now the gold standard for staging, detecting occult disease, and determining eligibility for PRRT; always obtain before initiating PRRT
- The distinction between well-differentiated G3 NET and poorly differentiated NEC is critical: G3 NETs may respond to PRRT and temozolomide-based regimens, while NEC requires platinum-based chemotherapy; pathologic review by an expert NET pathologist is essential
- Carcinoid crisis can be fatal during surgery, biopsy, or chemoembolization; prophylactic octreotide infusion (50-100 mcg/h IV) should be administered before any invasive procedure in patients with carcinoid syndrome
- Carcinoid heart disease (right-sided valve fibrosis from serotonin) occurs in 20-40% of carcinoid syndrome; screen with echocardiography at diagnosis and annually; 24h urine 5-HIAA >300 umol/day is a risk factor
